State Legislation, Regulations, and Hospital Guidelines for Newborn Screening for Critical Congenital Heart Defects — United States, 2011–2014

重大な先天性心疾患の新生児スクリーニングに関する州の法律、条例および病院のガイドライン ― アメリカ、2011～2014年

重大な先天性心疾患(CCHD)は生産児1,000名あたり約2名に発症する。新生児スクリーニングにより罹患率および死亡率を低下させることから、2011年9月、U.S. Department of Health and Human ServicesはRecommended Uniform Screening Panel (RUSP)にCCHDを追加、CDCは2014年、アメリカ小児科学会のDivision of State Government AffairsやAssociation of Public Health Laboratories のプログラムであるNewborn Screening Technical Assistance and Evaluation Program (NewSTEPs)と共同で、州別の新生児CCHDスクリーニング検査の採用状況について調査を行った。その結果、43の州にCCHD新生児スクリーニングを支援する法律、条例または病院ガイドラインがあり、13州にCCHDスクリーニング検査に関する条例があった。インディアナ州、メリーランド州およびニュージャージー州ではRUSPにCCHDが追加された2011年よりも前に法律が施行されており、25の州が2013年にCCHDスクリーニング検査を採用していた。また、これら43州のうち、データ収集は24州にて行われ、14州にてデータ収集が計画されているが、収集されるデータの種類は州により異なっていた。CCHDスクリーニング検査の普及により、早期診断および治療が可能となり、CCHDの罹患率および死亡率が抑止されると思われ、また、州レベルでのデータ収集、臨床転帰のモニタリングおよびスクリーニングプログラムの評価はCCHDサーベイランスに重要な要素である。
