Tetanus - Puerto Rico, 2002

2002年2月から5月、Puerto Rico Department of Health(PRDOH)は破傷風患者3例の報告を受けた。症例1 : 86歳男性。2001年12月19日、右手にとげが刺さった。12月29日に会話、嚥下、呼吸が困難となり、その4日後には頚部硬直と呼吸不全が発現して破傷風と診断された。破傷風およびジフテリアトキソイド(Td)の投与を受けたが、2002年2月2日に死亡した。症例2 : 68歳男性。2002年4月18日、さびた釘で足に刺傷を負った。4月22日に嚥下困難や軽度の息切れ、下顎硬直などが発現し、開口障害や筋硬直、会話困難などの所見より破傷風が疑われた。抗生物質(メトロニダゾール、CPFX、ミダゾラムなど)による治療とともに破傷風免疫グロブリン(TIG)の投与を行ったが、4月27日に死亡した。症例3 : 76歳男性。2002年4月10日、右手にとげが刺さった。4月18日に脱力感と嚥下困難が発現し、その後開口障害が発現して歩行や会話も困難となり、破傷風と診断された。4月22日にTIGの投与を受け、その後回復した。3例とも破傷風ワクチン接種歴は不明であった。1990年1月から2002年4月にPRDOHは20例の破傷風患者の報告を受けた。このうち18例(90%)は男性で、年齢は55－86歳、死亡率は68%であった。全例が破傷風ワクチン接種歴不明であり、5例(25%)は糖尿病歴があった。全ての創傷には破傷風のリスクがあるため、健康管理者は全ての創傷患者のワクチン接種状況をレビューする必要があり、また十分な効果が維持されるように破傷風トキソイドワクチンを投与するべきである。
