メジナ虫症の世界的根絶への進展、2014年1月~2015年6月
メジナ虫症(ギニア虫症)は寄生虫Dracunculus medinensisによる疾患であり、汚染した飲料水から感染した後、約1年で皮膚(主に下肢)に寄生虫が出現し、疼痛と二次性細菌感染症により労働や教育を中断させる一時的または永続的障害を来す。CDCは1980年、世界的メジナ虫症根絶キャンペーンを開始、1986年には世界保健総会がメジナ虫症根絶を呼びかけ、Carter Centerの主導およびWHO、UNICEF、CDC、その他のパートナーの支援による世界的なメジナ虫症根絶プログラムにより、メジナ虫症を風土病とする国々の保健省への援助が始まった。1986年の時点ではアフリカおよびアジアの20カ国において毎年推定350万例に発症していたが、2015年の年間報告例は99%減少し、風土病とする国は4カ国となっている。ここでは2014年1月~2015年6月におけるメジナ虫症根絶への進捗状況について報告する。2014年の症例数は126例(チャド:13、エチオピア:3、マリ:40、南スーダン:70)であり、2013年の148例(チャド:14、エチオピア:7、マリ:11、南スーダン:113、スーダン:3)から15%減少し、また、2015年1~6月の症例数は8例(チャド:6、エチオピア:1、南スーダン:1)となり、2014年1~6月の27例(チャド:6、エチオピア:2、南スーダン:19)から70%減少していた。メジナ虫症は、1)風土病とする国々に住む人々への教育、2)汚染した飲料水の布またはパイプフィルターによる濾過、3)汚染した表面水の殺虫剤テメホスによる殺虫処理、4)ボーリングまたは手掘りによる井戸からの安全な飲料水の供給により予防することが可能であり、伝染の封じ込めは、1)飲料水源の汚染を避けるための患者の隔離、2)二次感染を避けるための応急処置の提供、3)寄生虫の用手抽出、4)密封包帯の使用により可能であり、ワクチンおよび治療薬は現存しない。
