Combined Use of Inactivated and Oral Poliovirus Vaccines in Refugee Camps and Surrounding Communities — Kenya, December 2013

難民キャンプおよび周辺地域社会における不活化および経口ポリオウイルスワクチンの併用 ― ケニア、2013年12月

1988年に始まった世界ポリオ撲滅イニシアティブ(GPEI)により、現在、野生株ポリオウイルス(WPV)が蔓延する国はアフガニスタン、ナイジェリア、パキスタンの3カ国のみとなっている。2013年4～12月、ナイジェリアにて発生したWPV1型(WPV1)によるアウトブレイクではソマリア、ケニア、エチオピアにてそれぞれ194例、14例、9例が発症、これを受けてケニアでは12月14～18日に不活化ポリオウイルスワクチン(IPV)と経口ポリオウイルスワクチン(OPV)を組み合わせた史上初のキャンペーンを実施した。このキャンペーンはソマリア難民キャンプおよびケニアとソマリアの国境付近に住む59カ月齢以下の乳幼児126,000名を対象に299の接種チーム(難民キャンプ:173、国境地域:126)により行われ、6週齢未満の乳児はOPVのみ、6週～59カ月齢の乳幼児はOPVに続きIPVが接種された。これら接種チームのうち、無作為に47チームを抽出し接種状況をチェックした結果、43チーム(91％)はスタッフ、ワクチン、対象となる乳幼児へのワクチン供給いずれも十分であったが、IPV接種方法(投与部位および投与量)を誤っていた医療従事者メンバーが47名中5名(11％)認められた。ワクチンの保存は44チーム(94％)にて適切であり、電子温度モニターが設置されていた42チームでは、2℃以下:12チーム(42％)、8℃以上:8チーム(19％)であった。ワクチンの費用はIPV:2.09ドル/回、OPV:0.14ドル/回であり、乳幼児1人あたりの接種費用はOPVのみの場合0.50ドル、OPV+IPVキャンペーンでは3.27ドルであった。接種による重篤な疾患、入院、死亡は報告されていない。このキャンペーンによる接種率は難民キャンプ:92.8％、周辺地域:95.8％であった。
