Implementation of New TB Screening Requirements for U.S.-Bound Immigrants and Refugees — 2007–2014

アメリカへの移民および難民に対する新しいTBスクリーニング検査実施の要求 ― 2007～2014年

アメリカ全体における結核(TB)症例数は20年以上にわたって減少し続けているが、他国を出生地とする症例は増加しており、2013年では64.6％が外国生まれの症例であった。また、アメリカには毎年約45万人の移民と5～7万人の難民が入国しており、CDCはこれらの移民および難民のTB感染の確認および治療法を開発した。1991年より、TBの診断は15歳以上を対象に胸部レントゲン検査にて行われ、TBが疑われる場合は喀痰塗抹標本の検査が行われていたが、2007年、CDCは診断方法として喀痰培養検査を採用し、陽性の場合は直接監視下療法(DOT)を行うこととした(CDOT TB TI)。CDCのDivision of Global Migration and Quarantine(DGMQ)によるこのプログラムは各国の指定医と連携して実施され、指定医の育成と指導により2007年4月9日にタイ(モンおよびビルマの難民)の難民スクリーニングプログラム、10月1日にメキシコおよびフィリピンの移民スクリーニングプログラムが実施された。その後、2011年には51カ国にて実施されるようになり、アメリカへの移民の70％がスクリーニング検査を受けた。2012年には指定医により約 1,100例がTBと診断され、症例の約60％が塗抹標本検査は陰性で培養検査にて陽性であった。また、14例が多剤耐性TBと診断された。以上、CDOT TB TIによりアメリカへの移民および難民における潜在的TB陽性例の診断および治療が可能となり、アメリカへの入国後1年以内のTB発症リスクが低下した。
