デング熱のアウトブレイク ―ミクロネシア連邦、2012~2013年
2012年9月26日に、ミクロネシア連邦(FSM)のコスラエ州出身の35歳女性が頭痛、筋肉痛、嘔吐、白血球減少症、血小板減少症のため入院し、迅速診断試験(RDT)では、デング熱ウィルス(DENV)非構造蛋白質-1(NS1)陽性であった。その後、次の週に7例以上のデング熱が疑われた患者にRDTを実施し、3例がNS1または抗DENVイムノグロブリンM(IgM)陽性であった。2012年10月の間、デング熱症例は増加し続け、10月下旬にWHOはコスラエ州の保健サービス部門のアウトブレイク対応チームを技術支援するために、疫学者を配置した。6例の血清検体を参照研究所に送付し、RT-PCR法および酵素免疫法で測定した結果、DENV-4陽性:1例、抗DENV IgM抗体陽性:4例が検出され、合計5例でDENV感染が確認された。2012年9月26日から2013年3月14日までに、コスラエ州立病院でデング熱を疑われた症例は729例、入院は242例で、コスラエ州人口のそれぞれ11%および3.7%であった。RDTを実施した728例のうち、173例(84.0%)がNS1陽性、41例(19.9%)が抗DEMV IgM陽性であった。5.5ヶ月間の人口1,000人あたりの累積発生率は、デング熱が疑われる症例:110.0、RDT陽性症例:31.1であった。デング熱アウトブレイクの影響を最も受けた世代は15~39歳、自治体はLeluであった。蚊の標本調査では、デング熱を媒介するAedes aegyptiおよびA. albopictusが同定され、Leluおよび他の3つの自治体では、 A. albopictusの幼虫が優勢で、古タイヤが最も一般的な繁殖場所であった。コスラエ州の保健サービス部門は、FSM Department of Health and Social AffairsおよびWHOの支援を受け、デング熱調査の強化、最適な臨床管理、病院の対応能力の分析、蚊の種および分布の迅速な調査と制御により、アウトブレイクに対応した。アウトブレイクは多大な医療および公衆衛生サービスを必要とするため、限られた人材および物的資源を他の重要な医療および公衆衛生活動から振り替える結果となる。医療資源が限られた環境の太平洋諸島では、調査活動の強化、臨床検査能力の確保、危機管理計画の整備により、今後のデング熱アウトブレイクに備える必要がある。
