Routine HIV Screening During Intake Medical Evaluation at a County Jail —　Fulton County, Georgia, 2011–2012

郡刑務所の入所時医学的検査における定期的HIVスクリーニングの導入
－ジョージア州フルトン郡、2011～2012年

ジョージア州アトランタのフルトン郡刑務所(FCJ)にて2011年1月1日～2012年3月15日、入所時の医学的検査にHIVスクリーニング検査が導入された。入所予定者39,073名のうち22,920名(59％)に検査が行われ、すでにHIV感染の診断を受けていた473名(2.1％)および同意の得られなかった人を除き、18,869名に対し口腔液HIV迅速スクリーニング検査の実施を求め、12,141名に実施された。検査に反応を示した120名(1.0％)に対し確証的検査を行った結果、52例の新規診断例が認められ全検査例の0.4％)、うち48例は口腔液迅速検査、2例は静脈穿刺(2011年8月)、2例は口腔粘膜塗布検体(2010年12月)による診断であった。症例は男性(47例、33.7±10.7歳)、女性(5例、33.3±12.2歳)、全例が非ヒスパニック系黒人であり、男性の38％(18例)は同性愛者であった。また、36例(69％)は以前にHIV検査を受けており(4ヵ月～4年前)、22例が2年以内、1例が4ヵ月前に陰性と診断されていた。また、留置期間は9例(17％)が48時間以内、30例(58％)が14日間以内と短期の症例が多かったが、出所後、地域において治療を開始または継続する体制を整えることでHIV感染の拡大を抑止しうると考える。
