Multistate Outbreak of Fungal Infection Associated with Injection of Methylprednisolone Acetate Solution from a Single Compounding Pharmacy － United States, 2012

2012年9月18日、Tennessee Department of Healthはテネシー通院外科診療所にて硬膜外ステロイド投与を受けた症例が46日後にAspergillus fumigates髄膜炎と診断されたとの警告を発表した。CDCおよびNorth Caroline Department of Health and Human Servicesとの共同調査が行われ、9月27日までにさらに8例(テネシー州:7、ノースカロライナ州:1)が確認された。この9例はいずれもマサチューセッツ州フレミングハムのNew England Compounding Center(NECC)にて調合された防腐剤無添加のメチルプレドニゾロン酢酸塩溶液(MPA)の硬膜外投与を1回以上受けており、使用されたMPAは3ロット(05212012@68、06292012@26、08102012@51)であった。この製剤は17,500バイアル(80mg/mL)が1mL、2mL、5mLバイアル包装にて23州75施設に分配され、末梢関節痛および腰痛の治療に使用されていた。NECCは9月26日にこれら3ロットの製剤を自主回収し、10月3日にはくも膜下腔内に投与される無菌性製剤、10月6日はその他のすべての製剤を自主回収した。CDC、FDAおよび州および地方の保健局による複数州に及ぶ症例調査では、10月10日の時点で10州にて137例が確認され、うち12例が死亡していた。70例を対象とした先行調査では、髄膜炎:64例(91%)、脳卒中(腰椎穿刺なし):2例(3%)、硬膜外膿瘍または骨髄炎:2例(3%)、複数症状:2例(3%)であり、年齢は68(23～91)歳、男性22人、女性48人であった。徴候/症状は頭痛:57例(81%)、発熱:24例(34%)、悪心:21例(30%)、羞明:7例(10%)が多く、脳脊髄液(CSF)中白血球数は1,299(13～15,400)/μL、グルコース:42(11～121)mg/dL、タンパク質:129(45～588)mg/dLであった。真菌感染症は26例(37%)にて認められ、うち14例にてExserohilumspp(13例)およびAspergillus fumigates(1例)が同定された。発症は8月18日が最も早く、最終投与から発症までの期間は15(1～42)日間であった。NECCによる自主回収後の発症例は認めていない。
