水痘サーベイランスの評価－一部の州、2000～2010年

アメリカでは1996年に水痘ワクチンの1回接種が小児予防接種スケジュールに追加されたが、その当時の全国受動的水痘サーベイランスのデータは予防接種プログラムの影響をモニタリングするには不十分であった。CDCは、National Notifiable Diseases Surveillance System(NNDSS)の2000～2010年のデータを分析し、このデータがワクチン接種の影響のモニタリングに適切であるかどうかを検討した。2000年には12州が水痘患者の報告を要求しており、このうち2州は受動的症例ベースサーベイランスを実施し、10州はCDC基準に適合した適切で一貫した報告(水痘患者を少なくとも人口10万人あたり1例、3年以上連続してNNDSSに報告)を行っていた。2010年には水痘患者の報告を要求した州は39州、受動的症例ベースサーベイランスを実施した州は38州、適切で一貫した報告を行っていた州は31州に増加した。CDC基準に適合した州における水痘の発症率(10万人あたり)は、2000年の43.2例から2010年には8.9例に低下した(低下率79.4%)。水痘発症率の低下率は、水痘ワクチン1回投与が推奨されていた2000～2005年は43.3%であったが、小児予防接種スケジュールに水痘ワクチンの2回目の接種が追加された2006年から2010年にかけては71.6%も低下した。年齢を含めた患者データを報告した州のデータによると、2000年(3州)から2010年(28州)までの水痘発症率の低下率は1～4歳群(69.7%)と5～9歳群(86.2%)が最も大きかった。この年齢群の小児は、2000～2005年に水痘ワクチンの1回接種、2006～2010年に2回接種が推奨されていた。以上、NNDSSを通してCDCに報告された州別の水痘サーベイランスデータは現在の水痘発症率の傾向のモニタリングに適していることが明らかになった。今後、サーベイランスシステムの持続的な強化と全ての州のサーベイランスシステムへの参加が必要である。
