麻疹の伝播増加とアウトブレイク－ヨーロッパ地域、2011年

WHOヨーロッパ地域(EUR)では、2010年の麻疹根絶目標に向けて2003～2009年にかなりの進展がみられたが、2009年後期以降麻疹ウイルスの伝播やアウトブレイクが急増している。2011年1月1日～10月26日にはEUR全体で26,074例の麻疹患者が報告された[西ヨーロッパ地域:21,724例(83.3%)、中央/東ヨーロッパ地域:3,570例(13.7%)、旧ソビエト連邦の新たな独立州:780例(3.0%)]。このうち12,882例(49.4%)は15歳以上であり、11,763例(45.1%)はワクチン未接種、11,825例(45.4%)は接種歴が不明であった。全体で7,288例(28.0%)が入院し、9例が麻疹に関連して死亡した。また麻疹のアウトブレイクは2011年1月1日～10月26日にEUR加盟53州中36州で115件発生した。患者数は21,177例であり、この期間中の全麻疹患者の81.2%を占めた。フランスでは最大のアウトブレイクが進行中であり、10月26日の時点で14,025例の麻疹患者が報告されている。EURにおける麻疹の伝播およびアウトブレイク増加の主因は、様々な年齢の麻疹感受性集団における低いワクチン接種率である。2010年9月、第60回Regional Committee for Europeにより2015年までの麻疹根絶が新たな目標として設定された。この目標達成には、様々な年齢集団における麻疹含有ウイルス(MCV)の2回接種率＝95%の達成・維持、有効なアウトブレイク制御対策の実施、麻疹患者およびアウトブレイクを迅速に同定するためのサーベイランスのさらなる強化、麻疹根絶の確認が必要である。
