Evaluation of Automated Molecular Testing Rollout for Tuberculosis Diagnosis Using Routinely Collected Surveillance Data — Uganda, 2012–2015

結核診断のための自動分子遺伝学的検査の導入に関する定期的収集サーベイランスデータを用いた評価 ― ウガンダ、2012～2015年

2012年、ウガンダは高感度自動リアルタイムPCR検査システムGeneXpert MTB/RIFを導入し、2016年2月時点で国内111県のうち76県(68％)に111台を整備した。この検査システムは結核(TB)菌群遺伝子とリファンピシン(RIF)耐性遺伝子の変異を2時間以内に検出することが可能であり、WHOは本システムをHIV関連TBまたは多剤耐性TB感染が疑われる患者の1次診断に用いることを推奨している。TBの治療とコントロールにおけるこのシステムの有用性を、定期的に収集したサーベイランスデータを用いて導入前後で比較し、評価した。2012～2015年に10県においてNational Tuberculosis and Leprosy Programに登録されたTB患者の新登録率は、全体としてシステム導入前後で有意差は認められなかった。一方、4県においては細菌学的に確定診断されたTB症例の割合は有意に上昇した。2012～2015年に首都カンパラの6施設に登録された1,650例の症例経過データを用いた解析では、システム導入後、1施設において細菌学的に確定診断されたTB症例の割合およびTB治療を完了した患者の割合は有意に上昇し、治療を完了せず中断した患者の割合は有意に低下した。今回の評価では、本システムの有用性は一部しか示されなかったが、今後、全国的に本システムが導入された際には、より総合的な評価を行うべきである。
