Advisory Committee on Immunization Practices (ACIP)は季節性インフルエンザワクチンの使用に関する2013年の勧告を改訂した。2014～2015年のインフルエンザシーズンのために改訂された情報には、1)アメリカでの季節性インフルエンザワクチンの抗原組成、2)生後6カ月～8歳児に対するワクチン用量の検討、3)弱毒化インフルエンザ生ワクチン(LAIV)が直ちに利用可能な場合に健康な2～8歳児に対する使用の優先性が含まれる。ACIPは生後6カ月以上で禁忌のないすべての人に毎年のワクチン接種を推奨している。2014～2015年のインフルエンザワクチンは2013～2014年と同じウイルス株になると考えられ、三価ワクチンではA/California/7/2009(H1N1)-like株、A/Texas/50/2012(H3N2)-like株、B/Massachusetts/2/2012-like(Yamagata lineage)株から構成され、四価ワクチンではこれらの株に加え、B/Brisbane/60/2008-like(Victoria lineage)株から構成される予定である。生後6カ月～8歳では、免疫反応の最適化のために、初回のシーズン中にワクチンの2回接種(最低4週間隔)が必要とされている。しかし、2014～2015年のワクチン組成は2013～2014年と同じであるため、以前に2013～2014年のワクチンを1回以上接種している小児では、2014～2015年の必要とされる接種回数は1回のみである。LAIVおよび不活化インフルエンザワクチン(IIV)の有効性を比較した複数の試験において、成人では同等またはIIVのほうが有効であるという結果が得られているが、小児では数件の試験においてLAIVのほうが有効性の高い結果となった。このため、ACIPは乳幼児(生後6カ月～71カ月)の検査で確認されたインフルエンザに対し、LAIVはIIVよりも有効であると結論づけた。2014～2015年シーズンに対するACIPの勧告では、禁忌または接種上の注意を要さない2～8歳の健康な小児に対し、直ちに利用できる場合はLAIVを使用するべきであり、直ちに利用できない場合はIIVを使用するべきとしている。また、ACIPは卵アレルギーを有する人のワクチン接種に関し、調理した卵であればアレルギー反応なく摂取可能な人には通常のプロトコールで接種し、卵または卵を含有した食品を摂取後に蕁麻疹のみを経験した人には、製造に孵化卵を使用しない三価リコンビナントインフルエンザワクチン(RIV3)を接種 (18～49歳対象)、またはIIVを接種して30分以上経過観察することを推奨している。卵製品に対し心血管症状、呼吸窮迫、消化器症状、エピネフリン投与が必要な反応、救急介入が必要な反応を発症した経験のある人に対しては、18～49歳であればRIV3を摂取し、RIV3が使用できない場合および＜18歳または＞49歳では、重症アレルギーの治療経験のある医師によりIIVを接種し、30分間以上経過観察を行う。アレルギー反応への寄与が疑われる成分にかかわらず、過去にインフルエンザワクチンに対する重症のアレルギー反応がある場合は今後のワクチン接種は禁忌である。
