Licensure of 13-Valent Pneumococcal Conjugate Vaccine for Adults Aged 50 Years and Older

50歳以上の成人に対する13価肺炎球菌結合型ワクチンの認可

13価肺炎球菌結合型ワクチン(PCV13)は、2010年、FDAに認可され、Advisory Committee on Immunization Practices (ACIP)は浸潤性肺炎球菌性疾患(IPD)予防のため生後6週～71ヵ月齢の小児を対象に接種を推奨しており、現在は生後2ヵ月齢からの4回接種が推奨されている。2011年12月30日、FDAはPCV13を50歳以上の成人を対象に認可した。認可の根拠となったランダム化多施設試験はアメリカおよびヨーロッパで行われ、50歳以上を対象にPCV13およびPPSV23(23価肺炎球菌多糖体ワクチン)単回接種による免疫原性が比較された。ワクチン接種1ヵ月後、共通する12の血清型に対するオプソニン作用活性(OPA)アッセイによる幾何平均抗体価(GMTs)は、60～64歳にてPCV13はPPSV23と同等またはそれ以上であり、PCV13にのみ含まれる血清型6AのOPA抗体応答はPCV13にて高かった。50～59歳におけるOPA GMTsは60～64歳におけるGMTsと同等であり、70歳以上では同等またはそれ以上であった。約6,000例を対象に検討した安全性に関しては、PCV13、PPSV23ともに投与部位の疼痛、腫脹、投与腕運動制限、疲労感、頭痛、寒気、食欲低下、全身筋肉痛および関節痛などが認められ、重篤な有害事象の発現率は1ヵ月時にて0.2～1.7%、1～6ヵ月にてPCV13:1.2～5.8%、PPSV23:2.4～5.5%であった。2011年2月および6月のACIP会議では、PCV13の臨床的有用性およびワクチンに含まれる血清型による肺炎球菌性疾患の発症予防に関するデータがないことから、接種勧告はまだ出されておらず、現在では65歳以上および19～64歳の免疫低下例に対し、PPSV23の1回接種が推奨されている。PCV13に関しては2013年末までに現在検討中のデータが揃うことから、その時点で再度、接種勧告の見直しが行われる。
