Influenza Vaccination Coverage Among Health-Care Personnel － United States, 2010-11 Influenza Season

医療従事者におけるインフルエンザワクチン接種率－アメリカ、2010-11インフルエンザ期

The Advisory Committee on Immunization Practices (ACIP)およびHealthcare Infection Control Practices Advisory Committeeは国内すべての医療従事者(HCP)に対し、インフルエンザワクチンの接種を毎年行うよう推奨している。ここでは2010-11インフルエンザ期のHCPにおけるワクチン接種率に関するCDCのインターネット調査の結果を報告する。調査対象は1,931名、うち63.5%がワクチンの接種を受けており、病院勤務者で最も高く(71.1%)、長期療養施設:64.4%、開業薬局:64.1%、救急/外来:61.5%、歯科医院:54.6%、在宅看護:53.6%の順であった。職種別では医師/歯科医師(84.2%)、実地看護師/医師助手(82.6%)にて高く、看護師:69.8%、専門医療従事者:64.4%、技術者:64.0%と有意差を認め、年齢別では18～29歳:56.4%、30～44歳:57.8%、45～59歳:69.0%、60歳以上:74.2%と年齢に伴い高値を示した。ワクチンの利点に関しては、接種者と非接種者において大きな認識の相違が認められ、接種者では94.7%がワクチン接種は時間とお金をかける価値があると回答したのに対し、非接種者は45.8%であった。また、接種者では89.1%が接種により自分の周囲の人を守り、94.1%は自分がインフルエンザに罹らないとし、70.1%がインフルエンザは自分の健康に重大な脅威になると認識していた(非接種者ではそれぞれ44.6、54.2、34.2%)。さらに接種者の94.8%がワクチン接種は安全であると回答し、非接種者は66.2%であった。雇用者から接種を受けるよう要求されていたHCPは約13%であり、うち98.1%が接種を受け、要求されていなかったHCPの接種率は58.3%であった。Healthy People 2020ではHCPにおけるインフルエンザワクチン接種率の目標値は90%とされており、患者への感染を防ぐためにも今後、この目標値の達成が重要である。
