Human Jamestown Canyon Virus Infection － Montana, 2009

ヒトジェームズタウンキャニオンウイルス感染症－モンタナ州、2009年

ジェームズタウンキャニオンウイルス(JCV)はブニアウイルスのカリフォルニア血清型に属し、蚊を媒介して人畜共通感染症を来す。北米に広く分布するがヒトでの感染は稀であり、2004年に報告義務が課せられてからの報告数は15例である。今回報告する症例はモンタナ州にて、初めて報告された症例である。2009年5月26日、51歳の男性が発熱、頭痛、めまい、左半身のしびれ感および刺痛のため救急外来を受診した。血圧の上昇(214/119mmHg)を認めたが、心電図、脳MRIおよびCT検査に異常はなく、酸素投与と高血圧に対するテルミサルタンが処方され帰宅した。1週間後(6月2日)、発熱と頭痛が持続し、また、筋肉痛と脱力感を来しプライマリケア医を受診、神経性疾患が疑われたため、7月6日に神経科医を受診したが、複雑な片頭痛と診断され、頭痛薬が処方された。その後、症状は改善し、6ヶ月間症状は現れなかった。プライマリケア医を受診した際、モンタナ州郊外の自宅付近で蚊に刺されたと報告したため、血清検体を採取し検査したところ、ウエストナイルウイルス(WNV)特異IgMおよびIgG陽性を示したため、CDCアルボウイルス診断研究室にてWNV、JCV、St.Louis脳炎ウイルス(SLEV)およびLaCrosseウイルス(LACV)抗体検査が行われた。その結果、酵素免疫測定法（ELISA)にてJCV特異IgM抗体陽性が認められたため、JCV感染症と診断された。発熱を伴う神経性疾患はWNV感染症の他にJCV感染症を疑い、適切な検査による診断および治療が重要である。
