Prevalence of Obesity Among Adults with Arthritis － United States, 2003-2009

関節炎成人患者における肥満罹患率－アメリカ、2003～2009年

アメリカでは20歳以上の成人のうち約7,250万人が肥満症、約5,000万人が関節炎であり、医療費は毎年それぞれ1,470億ドル、1,280億ドルと推定されている。関節炎患者の多くは肥満であり、関節炎の進行、活動の制限、能力障害、QOLの低下、全関節置換術および関節置換術後の臨床転帰の低下と肥満との相関が示されている。今回、CDCはBehavioral Risk Factor Surveillance System (BRFSS)の2003～2009年データから関節炎患者における肥満の罹患率を分析した。分析対象は2003年:264,864、2005年:356,112、2007年;430,912、2009年:432,607名であり、肥満はBMI＝30.0とした。その結果、関節炎患者における肥満率は非関節炎一般成人に比べ54%高く、2003年における州別肥満率の中央値は33.2%(コロラド州:25.1%～オハイオ州:40.1%)、2009年は35.2%(コロラド州:26.9%～ルイジアナ州:43.5%)であった。2003～2009年にかけて、DCでは19.2%減少し、ウイスコンシン州では26.2%増加、また、この期間に14州およびプエルトリコにて肥満率は有意に増加した。年齢補正肥満率が30%を超えた州は2003年:37州およびDC、2005年:42州、2007年:45州、2009年:48州で、40%を超えた州はそれぞれ、2州、0州、7州、12州であった。肥満は関節に過度の負担をかけ、炎症性サイトカインやアディポカインの増加による炎症反応により関節変性が進行する。わずかな体重の減少でも関節炎には有効であるため、食事や運動などの生活習慣の見直しと実践による肥満の予防が重要である。
