Maternal, Pregnancy, and Birth Characteristics of Asians and Native Hawaiians/Pacific Islanders － King County, Washington, 2003-2008

1997年、Office of Management and Budgetは人種/民族データの収集・報告のための連邦基準を改訂し、アジア/太平洋諸島系をアジア系とハワイ先住民/太平洋諸島系(NHPI)に分けた。ワシントン州はアメリカで3番目に大きいNHPIコミュニティがあることから、2003～2008年の人口動態統計データを用いてキング郡におけるアジア系とNHPIの母親および妊娠/出産の特徴を比較した。キング郡では2003～2008年に142,350人の出生が記録され、このうち2,442人の母親はNHPI、26,229人の母親はアジア系であった。NHPIの母親はアジア系に比べて青少年(15～17歳)が有意に多く(それぞれ2.4%、0.6%)、妊娠前の過体重(BMI25.0～29.9)/肥満(BMI＝30.0)が有意に多く(23.9%、19.5%/49.9%、7.6%)、妊娠中の喫煙率が有意に高く(9.8%、1.4%)、早産児(妊娠37週未満)が有意に多く(12.6%、9.7%)、高出生体重児(4,500g)が有意に多く(3.0%、0.6%)、周産期管理を妊娠後期にのみ受けた、あるいは全く受けていない割合が有意に高かった(15.8%、4.2%)。極低出生体重児(＜1,500g)の割合はNHPIとアジア系で有意差がなく(それぞれ1.3%、1.0%)、低出生体重児(＜2,500g)の割合はアジア系がNHPIよりも有意に高かった(7.5%、5.5%)。以上、アジア系とNHPIの母親および妊娠/出産の特徴には重大な違いがあることが明らかになった。アジア系とNHPIの健康データを分けてルーチンで報告することは、これら2集団の健康状態の違いの評価に有用と考える。
