治療薬不足により複雑になった結核患者の強制送還－ホンジュラス、2006年5月～8月

Division of Immigration Health Services(DIHS)はImmigration and Customs Enforcement(ICE)により拘留された違法移民に保健衛生サービスを提供している。ICE拘留者は活動性結核(TB)疾患のスクリーニングを受け、必要な場合にはTB治療が開始される。拘留中にTBと診断された患者の約84%は治療完了（通常最低6ヶ月間）前に出身国へ強制送還される（非感染性と決定された場合のみ）が、そのような患者は治療中断・未完了、薬剤耐性TB発現、他者へのTB疾患伝播のリスクが高い。また送還後に再度違法入国する場合もあるため、DIHSはこれら患者が出身国でTB治療を続けられるよう米国はじめ他の国の保健衛生局などと協力して支援活動を行っている。この報告は、2006年5月－8月にホンジュラスでファーストラインTB治療薬(イソニアジド、リファンピシン、エタンブトール、ピラジナミド)が不足したため、ホンジュラスへ強制送還される活動性TB疾患患者の治療完了支援策として米国とホンジュラスの保険衛生局が協働して行った活動内容を述べている。活動方針として、TB治療中の拘留者は2週間分の医薬品を持たせて送還し、個々の患者の残りの治療薬(1～5か月分)は送還時に直接ホンジュラスNational TB Program(NTP)に送ることにした。また送還者と医薬品の追跡を容易にするため、ホンジュラスTegucigalpaの国際空港で送還者がホンジュラスNTP職員と会い、保健衛生サービスの受け方などの説明を受けるようにした(meet-and-greetプロセス)。この期間（5月23日－8月8日）中、活動性TBのホンジュラス人拘置者30例中16例が強制送還され、このうち2例は非マイコバクテリウムTBのため治療を続けなかったが、残り14例中13名(93%)は治療を完了した。1例は治療完了の1週間前に追跡調査ができなくなった。TBコントロールを成功させるには、公衆衛生分野外での協力関係を含め多数の関係機関の協同と支援が必要である。
