Estimated Current and Future Congenital Rubella Syndrome Incidence with and Without Rubella Vaccine Introduction — 19 Countries, 2019–2055

風疹ワクチンを導入した場合と導入していない場合に推定される現在および将来の先天性風疹症候群の発生率－19カ国、2019年～2055年

風疹は、ワクチンで予防可能な先天性異常の主な原因である。妊娠早期における風疹ウイルス感染は、流産、胎児死亡、死産、あるいは先天性風疹症候群(CRS)として知られる一連の先天性異常を引き起こす可能性がある。今回、風疹含有ワクチン(RCV)を国内の小児予防接種スケジュールに導入していない各国において、現在および将来の推定されるCRS発生率を調査した。さらに、たとえ麻疹含有ワクチン(MCV)の接種率が人口の80％未満でも、これらのプログラムにRCVを導入するという最近の勧告を実施した場合に推定される効果について検討した。2000年～2022年の期間にRCVを導入した国は、2000年の191カ国中99カ国(52％)から、2022年には194カ国中175カ国(90％)に増加した。2023年末までに、19カ国の低所得国および中所得国はRCVをまだ導入しておらず、このうち15カ国はWHOのアフリカ地域、4カ国は東地中海地域に属し、13カ国は低所得国、12カ国は紛争地域または制度が脆弱な国に分類されている。2019年に、これらの国では推定で24,000件のCRS症例が発生し、これは世界全体での推定32,000件の症例の75％に相当する。コンゴ民主共和国、エチオピア、ニジェール、ナイジェリア、スーダンの5カ国では、それぞれ約1,000件のCRS症例が発生したと推定された。2025年～2055年の期間に、これらの国でRCV導入の影響を予測するモデリング研究では、RCVがない場合、推定103万件のCRS症例が発生すると予測される。一方、2025年に19カ国でRCVが導入された場合、両方のワクチン接種シナリオ(MR接種率60％および90％)において、2025～2027年にかけてCRS症例の推定年間数は急激に減少し、2055年まで低水準を維持する可能性がある。2055年までに、両方のワクチン接種シナリオにおける予測される累積平均症例数は4万～6万件と推定される。2025～2055年の期間では、これらの19カ国での風疹ワクチン接種はRCVが導入されない場合と比較して、推定で98万6千件のCRS症例を予防できる可能性がある。これらの推計に基づき、WHOのStrategic Advisory Group of Experts on Immunizationは2024年9月に80％以上のMCV接種率という基準を撤廃し、これらの国での普遍的なRCV導入を勧告した。これら19カ国において、2025年～2030年にRCVが導入された場合、世界的な風疹およびCRS根絶に向けた進展が急速に加速する可能性がある。
