First Clade Ib Monkeypox Virus Infection Reported in the Americas — California, November 2024

アメリカで初めて報告されたクレードⅠbエムポックスウイルス感染症 ― カリフォルニア州、2024年11月

コンゴ民主共和国ではクレードIエムポックスウイルス(MPXV)のアウトブレイクが進行中であり、アフリカ以外の国でも旅行に関連したクレードI MPXV感染が報告されている。2024年11月、カリフォルニア州サンマテオ郡保健局は、東アフリカから最近帰国した男性旅行者のクレードI MPXV感染を示唆するPCR結果の電子検査報告を確認した。患者は民間航空機でアメリカに帰国中(day0)に発熱を自覚し、帰国後のday2に、発熱、息切れ、疲労、悪心、片側鼠径リンパ節肥大などの進行性の徴候および症状から、2カ所の緊急治療施設で診察を受けたが、渡航歴については明らかにされなかった。day4に発熱、悪寒、頭痛が持続するため救急科を受診し、検査で鼠径部蜂巣炎と急性腎障害が疑われ、入院となった。腕、胸部、腹部に「小さな隆起」のような掻痒性皮疹が出現したが、皮疹は当初、薬剤関連と考えられ、2.5％ヒドロコルチゾンクリームを局所塗布後に改善した。day6に患者は退院した。翌日、鼠径部の痛みが悪化し、顔、背中、腕、脚に多発性の丘疹、小水胞、膿疱病変が出現したため、他の救急科を受診した。この受診で、初めて東アフリカへの渡航歴が判明した。患者の腕と背中の膿疱からスワブ検体を採取し、水痘・帯状疱疹ウイルス(VZV)とMPXVの検査のため民間検査機関に送付した。day11に検査機関が天然痘以外のオルソポックスウイルス(NVO)DNAを検出し、クレードⅡ MPXV DNAはなかったことを報告したことから、クレードI MPXVの可能性が示唆された。VZVの結果は陰性であった。カリフォルニア州公衆衛生局(CDPH)と協議後、郡保健局の職員が同日、患者の自宅を訪問し、迅速なクレードI MPXV検査のために皮膚膿疱スワブ検体を採取した。翌朝、CDPH Viral and Rickettsial Disease LaboratoryでクレードI MPXVのDNAが確認された。患者は、東アフリカ旅行中に社交的な集まり参加し、全身マッサージを受けたが、旅行中に性的接触はなかったと報告した。患者は、day23に地元保健所の医師による臨床検査ですべての病変が消失したことが確認されるまで自宅で隔離された。本症例は、アメリカ大陸で報告された最初のクレードI MPXV感染例であった。83名の接触者が特定され、5名(家庭内接触者1名と旅行同行者4名)が曝露後予防としてJYNNEOSワクチンを接種した。すべての接触者は21日間監視され、二次感染例は確認されなかった。エムポックスと一致する病変または臨床的特徴のある患者はMPXV検査を受ける必要がある。医療従事者は追加検査のきっかけをつくり、感染経路別予防策やその他の予防的公衆衛生介入の迅速な実施を促進するために、クレードI MPXV感染が疑われる(エムポックスの症状、クレードI MPXVの伝播が進行中の地域への旅行歴など)またはNVO DNA が検出され、クレードⅡ MPXV DNA が未検出の検査結果を公衆衛生当局に直ちに通知する必要がある。
