ポリオ根絶に向けた進展 ― アフガニスタン、2023年1月～2024年9月

1988年にGlobal Polio Eradication Initiativeが開始されてから、野生株ポリオウイルス(WPV)2型および3型は根絶した。ポリオの年間症例数は99.9％超減少したが、WPV1型(WPV1)は依然としてアフガニスタンとパキスタンで流行している。今回、2023年1月～2024年9月にアフガニスタンにおけるポリオ根絶へ向けた予防接種およびサーベイランス活動と進捗状況について報告する。アフガニスタンの1歳児における2価経口ポリオワクチン(bOPV)の3回接種率は、2021年が56％、2022年が61％であったのに対し、2023年は68％と推定された。不活化ポリオワクチンの2回接種率は2021年が30％、2022年が43％であったのに対し、2023年は45％と推定された。2023年、5歳未満の幼児を対象としたbOPVの追加接種活動(SIA)は13回実施された[全国予防接種日(NID)3回、地方予防接種日(SNID)5回、症例対応キャンペーン5回]。3回のSIAでは東部の州にて対象年齢が拡大された(10歳未満)。NID、SNID、症例対応キャンペーンによる接種率はそれぞれ77％～98％、88％～97％、81％～120％であった。2024年1月～7月にはNIDが2回、SNIDが3回行われ、4回のSIAは隣国パキスタンと同時に行われた。2024年1月～5月のSIAでのbOPV接種率は88％～97％であり、6月に約7年ぶりに全国的な戸別訪問接種について当局から発表され、地区での戸別訪問接種はそれまで72％～85％であったが、6月、7月には96％～99％まで増加した。しかし、9月に当局は全国的な戸別訪問から固定施設での接種様式に変更した。2023年のキャンペーン後のモニタリング調査による接種率は南部では73％～85％、北東部では80％～87％、その他の地区では90％～100％であり、固定施設にて68％～75％、モスクにて68％～81％、戸別訪問にて90％～97％であった。2024年には南部以外のすべての地域で接種率91％～99％に増加したが、南部地域では戸別訪問接種が許可された6月より前は19％～77％、6月、7月は89％～91％であった。OPVはパキスタンとの主な国境検問所であるトールハム(東部地域)、フレンドシップゲート(南部地域)から入国するすべての年齢の人にも接種され、2023年1月～2024年9月、約170万人が接種した。2023年における非ポリオ急性弛緩性麻痺率は、15歳未満の小児10万人あたり26(17.0～39.1)、便検体の適正率は94％(90.7％～97.0％)、2024年1～9月ではそれぞれ23.7(15.2～36.8)、95％(91.8％～98.0％)であった。環境サーベイランスでは2023年に4地域8州の62検体よりWPV1が検出され[東部：Nangarhar州(42検体)、Kunar州(5検体)、南部：Kandahar州(8検体)、Zabul州(2検体)、Helmand州(1検体)、Uruzgan州(1検体)、中央部：Kabul州(2検体)、北部：Balkh州(1検体)]、2024年1月～9月に5地域10州の85検体よりWPV1が検出された[南部：Kandahar州(35検体)、Helmand州(22検体)、Uruzgan州(2検体)、東部：Nangarhar州(13検体)、Laghman州(3検体)、Kunar州(1検体)、南東部：Paktya州(1検体)、Ghazni州(1検体)、中央部：Kabul州(5検体)、西部：Hirat州(2検体)]。WPV1症例は2021～2022年に南東部と北東部より6例報告され、2023年には東部Nangarhar州より6例報告された。2024年1月～9月には23例のWPV1症例が報告され、過去4年間で最多となっており[東部のNuristan州1例、Kunar州1例、南部のKandahar州14例、Helmand州6例、Uruzgan州1例]、平均年齢は39カ月齢(8～120カ月齢）、23例中3例は定期予防接種やSIAを通じてのOPV接種歴がなく、17例はSIAを通じてOPVを接種(平均8回)、17例は定期予防接種にてOPVを受けていた(平均3回)。データでは戸別訪問のワクチン接種が最も効果的なキャンペーン法であることが示されているが、アフガニスタンにおける当局による現在のアプローチは戸別訪問以外の方法に限定しており、現在感染の中心となる南部地域での感染を阻止するためにアフガニスタンとパキスタン協同でのポリオプログラムのさらなる強化が求められる。
