Progress Toward UNAIDS Global HIV Pre-Exposure Prophylaxis Targets: CDC-Supported Oral Pre-Exposure Prophylaxis — 37 Countries, 2017–2023

UNAIDSの世界のHIV曝露前予防目標に向けた進展：CDCが支援する経口曝露前予防投与－ 37カ国、2017年～2023年

経口の曝露前予防(PrEP)を推奨通りに服用した場合、性交渉によるHIV感染リスクを99％、静注ドラッグ使用によるHIV感染リスクを74％以上減少させる。Joint United Nations Programme on HIV/AIDS(UNAIDS)は2030年までに公衆衛生上のHIVの脅威を終結させるため、2025年中に全世界で2,120万人のPrEP使用(開始または継続)を達成するために医療サービスを拡大するという目標を設定している。アメリカ大統領エイズ救済緊急計画(PEPFAR)では国防総省、国際開発庁、CDCを含むアメリカ政府機関とともに、ホスト国とのパートナーシップを通じてPrEPを支援しており、2016年から15～24歳の女性に最初に着目してPrEPを世界的に実施し、2020年にはPERFARが支援する多くの国が経口PrEPを予防活動に組み入れている。CDCはサハラ以南のアフリカ4地域、37カ国にてPrEPプログラムを支援している[サハラ以南のアフリカ地域(19カ国)、東南アジア地域(5カ国)、ユーラシア地域(4カ国)、アメリカ地域(9カ国)]。今回、2017年～2023年における優先集団に対するCDCが支援するPrEP開始について地域別、性別、年齢別に報告する。CDCが支援するPrEP開始は37カ国にて2017年の11,397名から2023年には856,816名に増加し、7年間にて合計約230万人となり、目標値を上回った(118.7％)。年ごとのPrEP目標値の年間達成率は81.4％(2020年)～141.6％(2022年)であり、2023年は全4地域にて100％を超えた。地域別では、CDCが支援するPrEPのほぼすべて(96.0％、2,187,946名)がサハラ以南のアフリカ地域で開始され[東南アジア(1.9％、42,177名)、ユーラシア地域(0.8％、19,049名)、アメリカ地域(1.3％、29,571名)]、2017年の11,302名から2023年には820,777名へ増加した。前年比増加が最大であったのはアフリカ地域では2020年から2021年(220％増)、東南アジア地域、ユーラシア地域では2021年から2022年(それぞれ67％、214％増)、アメリカ地域では2022年から2023年(67％増)であった。性別に関してはアフリカ地域では女性が多く(65.8％)、他の地域では男性が多かった(東南アジア地域85.2％、ユーラシア地域73.8％、アメリカ地域77.6％)。年齢別ではアフリカ地域および東南アジア地域では15～24歳(それぞれ44.9％、42.9％)、ユーラシアでは35～44歳(31.7％)、アメリカでは25～34歳(46.7％)が最も多かった。PrEP開始は、アフリカ地域では女性の性労働者(63.4％)、男性同性間性的接触者(MSM：25.6％)にて多く、他の地域ではMSMにて最も多く(東南アジア地域93.4％、ユーラシア地域69.2％、アメリカ地域77.2％)、全体ではトランスジェンダー(1.1％)、刑務所やその他の閉鎖的な環境にいる人(3.2％)、静注ドラッグ使用者(6.0％)にて低かった。UNAIDSによれば2023年のPrEP使用は計350万人と推算され、2025年の目標値2,120万人を達成するには大幅な増加が必要である。
