Two Outbreaks of Legionnaires Disease Associated with Outdoor Hot Tubs for Private Use — Two Cruise Ships, November 2022–July 2024

個室で利用する屋外温浴浴槽に関連したレジオネラ症の2件のアウトブレイク ― 2隻のクルーズ船、2022年11月～2024年7月

レジオネラ症レジオネラ属菌が原因の重篤な肺炎である。2022年11月～2024年6月、CDCは2隻のクルーズ船(A船、B船)の乗客におけるレジオネラ症12例の通知を受けた。A船では、2022年11月～2024年4月に利用した乗客8例のレジオネラ症がCDCに報告された。下気道検体は入手できず、6例が入院したが、死亡例はなかった。B船では、2024年1月～6月に利用した乗客4例のレジオネラ症がCDCに報告された。4例とも入院したが、死亡例はなかった。CDCはクルーズ客船会社と連携して、各クルーズ船で関連の可能性がある2番目の症例が通知された後、船内での感染源の可能性を究明するための調査を開始した。患者へのインタビュー調査、環境評価、サンプリングから疫学データを収集した結果、特定の客室バルコニーにある個室の温浴水槽が最も可能性の高い感染源であることが判明した。レジオネラ属菌の増殖を最小限に抑えるため、両クルーズ客船会社は、発熱体を取り除き、使用の合間に水を抜き、塩素処理と清掃の頻度を増やすなど、これらの装置の運用とメンテナンスを変更した。温浴浴槽は、設置場所に関係なく、メンテナンスと運用が不適切な場合、レジオネラ属菌の増殖と感染に好都合な条件を与える。クルーズ船の個室の温浴浴槽には、共用エリアの温浴浴槽と同じメンテナンス要件は適用されない。クルーズ業界全体でアメニティとして提供される温浴浴槽タイプの装置の範囲を考慮すると、レジオネラ属菌の増殖および感染のリスクを減らすために、クルーズ船の水管理プログラムのスタッフが個室のバルコニー温浴浴槽の一覧表を作成して評価し、共用温浴浴槽のメンテナンスおよび運用プロトコルを個室の屋外温浴浴槽利用に適合させることが重要である
