キンダーガーテンの幼児における特定ワクチンの接種率と免除率 ― アメリカ、2023-24学年度

アメリカでは州や地方自治体が就学の要件としてワクチン接種を義務付けており、親が幼児の予防接種または接種免除の書類を学校に提出するか、学校が州の予防接種情報システムから記録を入手する。州は毎年、CDCにキンダーガーテンの幼児の予防接種および接種免除の人数を報告している。今回、2023-24学年度のモンタナ州を除く49州およびワシントンDCから報告されたデータから、キンダーガーテン入学児における予防接種率および免除率を分析した。麻疹・おたふくかぜ・風疹混合ワクチン(MMR)2回接種率は92.7％であり[79.6％(アイダホ州)～98.3％(ウェストバージニア州)]、11管轄地域にて95％以上、14管轄地域にて90％未満であった。ジフテリア・破傷風・無細胞百日咳ワクチン(DTaP)5回接種率は92.3％であり[79.5％(アイダホ州)～98.4％(ウェストバージニア州)]、12管轄地域にて95％以上、15管轄地域にて90％未満であった。ポリオワクチン4回接種率は92.6％であり[80.1％(アイダホ州)～98.4％(ウェストバージニア州)]、12管轄地域にて95％以上、13管轄地域にて90％未満であった。水痘ワクチン(VAR)2回接種率は92.4％であり[79.1％(アイダホ州)～99.7％(ウェストバージニア州)]、10管轄地域にて95％以上、15管轄地域にて90％未満であった。各ワクチン接種率は2022-23学年度に比較し、ほとんどの州において低下し、MMR、DtaP、ポリオワクチン、VAR接種率はそれぞれ35、32、33、36の管轄地域で低下した。接種が必須であるワクチン(MMR、DTaP、ポリオ、VARに限らない)が1種類以上免除された幼児は2022-23学年度の3.0％に対し、2023-24学年度は3.3％であり[0.1％未満(ウェストバージニア州)～14.3％(アイダホ州)]、非医学的理由による免除は報告された免除の93％以上を占め、全国の免除増加分のほぼ100％を占めていた。ワクチン接種免除率は30州にて3.3％を上回り、14州にて5％を超えていた。MMR接種率95％以上を達成できなかった管轄地域は2020-21学年度では2カ所であったが、2023-24学年度では14カ所となった。キンダーガーテンへの仮入学または猶予期間にある幼児の割合は、これらのデータを報告した31州にて2.6％であった[0.3％(デラウェア州)～8.0％(アーカンソー州)]。キンダーガーテン幼児が入学時に確実にワクチン接種を完全に受けているようにするため、保健所、学校、医療提供者による取り組みが必要である。
