Use of Haemophilus influenzae Type b–Containing Vaccines Among American Indian and Alaska Native Infants: Updated Recommendations of the Advisory Committee on Immunization Practices — United States, 2024

侵襲性Haemophilus influenzae菌b型(Hib)感染症は、アメリカ先住民/アラスカ先住民(AI/AN)の集団に偏って影響を及ぼす重篤な細菌感染症である。髄膜炎菌血清群Bの外膜タンパク質複合体に結合したHib莢膜多糖類[ポリリボシルリビトールリン酸(PRP)]からなる一価Hib結合ワクチン(PRP-OMP、PedvaxHIB、Merck and Co社)によるHibワクチン接種は、侵襲性Hib感染症のリスクが高いAI/AN乳児に対して歴史的に推奨されてきており、これは、初回接種後に相当な防御が得られるためである。2024年6月26日、CDCのAdvisory Committee on Immunization Practices(ACIP)は、PRP-OMP Hib成分が含まれているため、ジフテリア・破傷風トキソイド・無細胞百日咳(DTaP)、不活化ポリオウイルス(IPV)、Hib結合体、B型肝炎(HepB)のワクチンを混合した6価のDTaP-IPV-Hib-HepB(Vaxelis、MSP Vaccine Company社)を、1価PRP-OMPとともにAI/AN乳児に対する優先的推奨に含めることを推奨した。AI/AN乳児には、1)1価PRP-OMP(生後2カ月と4カ月の2回接種)またはDTaP-IPV-Hib-HepB(生後2カ月、4カ月、6カ月の3回接種)のいずれかからなるHibワクチンのプライマリー接種シリーズが推奨される。DTaP-IPV-Hib-HepBは、2カ月齢、4カ月齢、6カ月齢の乳児にのみ適応されており、Hib、DTaP、IPVワクチンのブースター接種には使用するべきではない。Hibワクチンのブースター接種では、AI/ANの小児に対して推奨されるワクチン製剤はなく、任意のHibワクチン(DTaP-IPV-Hib-HepBを除く)を使用する必要がある。今回の報告では、これらの勧告について検討されたエビデンスを要約し、AI/ANの乳児および小児におけるHib含有ワクチンの使用に関する臨床ガイダンスを提供している。
