Human Papillomavirus Vaccination Coverage Among Adolescent Girls Aged 13–17 Years — U.S.-Affiliated Pacific Islands, 2013–2023

13～17歳の思春期女子におけるヒトパピローマウイルスワクチン接種率 ― アメリカ領太平洋諸島、2013年～2023年

世界中で子宮頸がんは女性のがんで4番目に多いがんであり、アメリカ領太平洋諸島(USAPI)が位置するWHO西太平洋地域は推定症例の1/4を占めており、マーシャル諸島は子宮頸がんの年齢調整子宮頸がん率が世界で最も高い(女性10万人あたり74例)。子宮頸がんはヒトパピローマウイルス(HPV)ワクチンにより約75％が予防されると推定され、CDCは11～12歳の男女にHPV接種を推奨している。WHOは2030年までに女子の90％以上が15歳までにHPVワクチン接種シリーズを完了することを目標としている。USAPIでは2007～2016年にHPVワクチンが導入され、主に学校での予防接種プログラムにより接種されている。今回、USAPIの6管轄地域のうち5管轄地域(アメリカ領サモア、北マリアナ諸島、ミクロネシア連邦、マーシャル諸島、パラオ)における2013年～2023年の思春期女子のHPVワクチン接種率について報告する。データは各管轄地域における予防接種情報システムの患者データを集計し、レトロスペクティブポイントインタイム分析により各年12月31日時点のワクチン接種率を推定した。13～17歳の思春期女子におけるHPVワクチンの1回以上接種率と接種シリーズ完了率は地域によって異なり、2023年12月では1回以上接種率は58.0％(パラオ)～97.2％(北マリアナ諸島)、ワクチン接種シリーズ完了率は43.4％(パラオ)～91.8％(北マリアナ諸島)であり、2023年の時点では北マリアナ諸島のみがWHOの2030年目標値に達していた。2013～2023年におけるHPVワクチンの1回以上接種率は35.2ポイント(北マリアナ諸島)～72.8ポイント(アメリカ領サモア)増加し、シリーズ完了率は35.3ポイント(パラオ)～72.9ポイント(アメリカ領サモア)増加した。パラオ以外では2020年に1回以上接種率が71.6％、シリーズ完了率が59.0％とピークに達したが、その後2023年にはそれぞれ58.0％、43.4％に減少した。パラオ以外の地域では2013年から2023年にかけて毎年増加し、アメリカ領サモアではおそらく2025年までにWHO 2030年目標値に達する。パラオを除く地域では、ワクチン接種の脱落率が毎年減少し、北マリアナ諸島では2023年の脱落率がわずか5.6％であった。パラオでは2021年に17.2％となったが、2023年には25.2％に増加していた。以上、ワクチン接種率の評価により目標値達成への進捗状況をモニターし、接種率の低い集団や地域の特定が可能となる。接種率を増加するため、これらのエビデンスに基づいた戦略の実施が有効となる。
