13～17歳の思春期齢における全国ワクチン接種率 ― National Immunization Survey-Teen、アメリカ、2023年

Advisory Committee on Immunization Practicesは11～12歳の小児に破傷風・ジフテリア・無細胞性百日咳ワクチン(Tdap)、4価結合型髄膜炎菌ワクチン(MenACWY)、ヒトパピローマウイルス(HPV)ワクチンの定期接種を推奨している。また、小児期のワクチン接種を逃した思春期齢もキャッチアップ接種、最新の状態でのCOVID-19ワクチン接種、年1回のインフルエンザワクチン接種を受けるべきである。今回、CDCは2015年～2023年のNational Immunization Survey-Teen(NIS-Teen)データを使用し、1)Vaccines for Children(VFC)プログラムの対象の有無と出生年別による接種率の傾向、2)13～17歳の思春期齢における2023年のワクチン接種率、3)出生年別の接種率における最近の傾向について評価した。2023年の接種率は2005年1月～2010年12月に出生した13～17歳16,568人のNIS-Teenデータを分析した。13歳までのTdap 1回以上、MenACWY 1回以上、HPVワクチン1回以上の接種率は、2007年生まれ(接種対象期間がCOVID-19パンデミック前)のVFC対象者と2008～2010年生まれ(接種対象期間がCOVID-19パンデミック期間)のVFC対象者と同等であったが、HPVワクチンの接種状態が最新である割合(HPV UTD)は2007年生まれが53.0％、2010年生まれが42.7％と10.3ポイントの差があった。VFC対象者とVFC非対象者における13歳までのTdap 1回以上、MenACWY 1回以上接種率は2003年～2008年生まれではVFC対象者にて低く、2009年および2010年生まれでは同等であった。HPVワクチン1回以上接種率およびHPV UTDは2002年～2005年生まれではVFC対象者にて高く、2006年～2010年生まれでは同等であった。2023年における思春期齢に推奨されるすべての定期予防接種の接種率は2022年と同等であり、2023年の思春期齢におけるTdap 1回以上接種率は89.0％、MenACWY 1回以上接種率は88.4％、HPVワクチン1回以上接種率は76.8％、HPV UTDは61.4％であった。また、2007年生まれと比較した2008年生まれの13歳までのTdap 1回以上、MenACWY 1回以上、HPVワクチン1回以上接種率はそれぞれ2.3ポイント、2.6ポイント、3.2ポイント低く、14歳および15歳までの定期ワクチン接種率も低いままであった。2009年生まれでは13歳および14歳までの接種率達成はCOVID-19パンデミック前と同等であった。2010年生まれでも13歳までの接種率はパンデミック前と同等であったが、HPV UTDのみ2007年生まれに比べ7.1ポイント低かった。以上、VFC対象の思春期齢におけるワクチン接種率はHPV UTDを除き安定しており、COVID-19パンデミック前はVFC対象者と非対象者にて接種率に差を認めたが、直近の出生年では同等であった。
