Use of Respiratory Syncytial Virus Vaccines in Adults Aged ≥60 Years: Updated Recommendations of the Advisory Committee on Immunization Practices — United States, 2024

RSウイルス(RSV)は、アメリカでは秋から冬にかけて高齢者の呼吸器疾患や入院の主な原因となっている。2023～2024年のRSVシーズンは、臨床での共同意思決定を使用して、アメリカの60歳以上の成人にRSVワクチン接種が推奨される初のシーズンとなった。2024年6月26日に、Advisory Committee on Immunization Practicesは推奨事項の更新を表明し、FDAが承認したRSVワクチン[アレックスビー(GSK社)、アブリスボ(Pfizer社)、mResvia(Moderna社)]の単回接種が75歳以上のすべての成人と重症RSV疾患のリスクが高い60～74歳の成人に推奨されている。以前にRSVワクチンを接種した成人は追加の接種を受ける必要はない。今回、ワクチンの有効性と安全性に関する認可後のデータを含む、更新された推奨事項のエビデンスの検討についてまとめ、60歳以上の成人におけるRSVワクチンの使用に関する臨床ガイダンスを提供する。mRNA RSVワクチンのmResviaは、明らかになっているすべての追跡期間(被験者1例あたり中央値18.8カ月)にて、2つ以上のRSVに関連した下部呼吸器疾患(RSV-LRTD)症状に対するワクチン有効性(VE)は47.4％、3つ以上のRSV-LRTD症状に対する有効性は48.4％であった(Moderna社、第2/3相臨床試験、未発表データ、2024年)。重度の反応原性事象は、mResvia接種者でプラセボ接種者よりも多かった。mResviaワクチン接種後42日以内におけるギラン・バレー症候群(GBS)、その他の炎症性神経系事象、心筋炎、心膜炎の症例の記録はなかった。タンパク質サブユニットRSVワクチン(アレックスビー、アブリスボ)の認可後のデータは、60歳以上の成人においてワクチン接種後の最初のRSVシーズン中のRSV関連入院に対するVEを検討した4つの観察研究を再調査し、一般集団または免疫正常の成人のみで推定したVEは75％～82％であった。VEはワクチン製品(アレックスビーとアブリスボ)、患者の年齢群(60～74歳および75歳以上)で同等であった。特定の免疫不全状態および末期腎疾患の60歳以上の成人においても有効性が示された。全体的に、この初期分析の結果では、高齢者におけるRSVワクチン接種に関連するGBSのリスク上昇の明確で決定的なエビデンスは示されなかったが、GBSのリスク上昇を否定することはできなかった。GBSの安全性サーベイランスは継続中で、追加の分析が進行中である。これらの更新された推奨事項は、最もリスクが高い人を明確にし、以前の臨床での共同意思決定の推奨事項に関連する実施の障壁を軽減することにより、RSVワクチン接種の恩恵を受ける可能性が最も高い人におけるワクチン接種率を最大化することを目的としている。認可後の継続的なモニタリングは将来の推奨事項の指南となる可能性がある。
