Varicella Outbreak Among Recent Arrivals to New York City, 2022–2024

ニューヨーク市へ最近の到着者における水痘のアウトブレイク、2022年～2024年

水痘は全身性掻痒性皮疹を特徴とする疾患であり、空気感染、飛沫感染、接触感染により感染する。ほとんどの人では軽症～中等症であるが、肺炎や死亡などの重篤な合併症を引き起こす可能性もある。水痘ワクチンは感染予防効果が高い。ニューヨーク市(NYC)は2022年春以降、約18万人の移民を受け入れ、現在、約65,000人が市の運営するシェルターや居住施設で生活している。2022年10月、NYC Department of Health and Mental Hygiene(DOHMH)は中央アメリカまたは南アメリカから、またはそこを経由して最近移住した居住施設の入居者における水痘患者3例を確認し、速やかに調査を開始した。2024年3月8日時点で、2022年9月12日～2024年3月6日に発症したアウトブレイク関連水痘患者が計873例確認された。患者の年齢中央値は11歳(生後2週～70歳)、17.5％が4歳未満、53.0％が4～18歳、29.4％が18歳より上であった。ほとんどの患者(802例、91.9％)は発症時点で水痘ワクチンの接種記録がなかった。28例が入院し、入院患者の年齢中央値は22歳(生後2週～43歳)、15例は合併症のため(脳炎、肺炎、菌血症、二次性細菌性皮膚感染症など)、3例は診断検査のため、10例は隔離または経過観察のための入院であった。感染時点で9例が妊娠しており、そのうち5例がNYCにて出産し、2例はNYC以外で出産し、2例はまだ分娩していない。死亡例は報告されていない。感染源が明らかな780例(89.3％)のうち、41.3％はシェルター/居住施設にて感染し、39.4％は輸入例/輸入の可能性例(潜伏期間の全部または一部がNYC到着前など)、18.2％は家庭内/家族からの感染であり、学校での感染は1.2％であった。患者はシェルター/居住施設105施設に居住し、施設あたりの患者数は中央値3例(1～197例)であり、1つの大規模施設(約950室)にて全体の約1/4の患者(197例、22.6％)が報告された。この施設でのアウトブレイクは拡大ワクチン接種キャンペーン後に終息したが、NYCへの感染者の輸入は続いている。約27,000回分の水痘含有ワクチンがDOHMHとNYC公立病院システム(NYC Health＋Hospitals)が配置したワクチンベンダーにより、最近移住した小児、青年期、成人に接種された。今回のアウトブレイクにより、高い水痘ワクチン接種率を達成し、維持することで感染を抑制することの重要性、シェルター/居住施設での輸入進行、感染に対する迅速で大規模なワクチン接種の必要性が強調された。
