Use of the Pfizer Pentavalent Meningococcal Vaccine Among Persons Aged ≥10 Years: Recommendations of the Advisory Committee on Immunization Practices — United States, 2023

10歳以上の人におけるファイザー社の5価髄膜炎菌ワクチンの使用：Advisory Committee on Immunization Practiceの推奨 ― アメリカ、2023年

髄膜炎菌による髄膜炎菌性疾患は生命を脅かす侵襲性感染症であり、CDCのAdvisory Committee on Immunization Practices (ACIP)は11歳または12歳に対し4価(血清群A、C、W、Y)髄膜炎菌結合型ワクチン(MenACWY)単回接種、その後16歳にて追加接種を推奨している。また、16～23歳に対し血清群B髄膜炎菌ワクチン(MenB)の2回接種シリーズ、さらに髄膜炎菌性疾患リスクの高い2カ月齢以上に対しMenACWY、10歳以上に対しMenBの定期接種を推奨している。今回、2023年10月に5価髄膜炎菌ワクチン(MenACWY-TT/MenB-FHbp)の10～25歳への使用が認可されたため、5価髄膜炎菌ワクチンの推奨事項のエビデンスと使用に関する臨床ガイドラインを示す。これらのエビデンスは10～25歳の健常人を対象に免疫原性と安全性を評価した3つのランダム化四重盲検試験のデータに基づくものであり、対象者は5価ワクチン群(6カ月または12カ月間隔にて2回接種)と対照群(MenACWY-CRM 1回接種+MenB-FHbp 6カ月間隔にて2回接種)に分けられた。初回接種1ヵ月後の血清群A、C、WおよびYに対する抗体応答は対照群に比べ5価群にて高く、抗体保有も5価群(MenACWY-TT/MenB-FHbp 2回接種後48カ月)にて対照群(MenACWY-CRM 1回接種後54カ月)に比べ高かった。血清群Bに対する抗体保有はMenB-FHbp 2回接種+MenACWY-CRM 1回接種後48カ月)と5価ワクチン2回接種後48カ月にてほとんど差は認めなかった。有害事象は5価群0.6％、対照群0.5％にて発現、MenACWY-TT/MenB-FHbpによる有害事象は注射部位痛の痛み(84.4％～89.3％)、頭痛(39.8％～46.8％)が多く、ほとんどが軽度～中等度であった。ACIPは10～25歳および髄膜炎菌性疾患リスクの高い10歳以上に対しMenACWY-TT/MenB-FHbpの接種を推奨し、投与間隔は6カ月としている。他のワクチンとの同時投与、妊婦および授乳婦への接種に関するデータは限られており、妊婦は利益がリスクを上回る場合を除き延期されるべきである。
