Federal Retail Pharmacy Program Contributions to Bivalent mRNA COVID-19 Vaccinations Across Sociodemographic Characteristics — United States, September 1, 2022–September 30, 2023

Federal Retail Pharmacy Program(FRPP)はCOVID-19パンデミックの緊急対応時にワクチン接種能を拡大するための国家の取り組みにて、パートナーとしての薬局の統合を促進した。アメリカでは約90％の人が地域の薬局から8km以内に住んでおり、薬局は、とくに低所得者層にてアクセスが容易な医療資源となっている。FRPPは連邦政府、アメリカの州および海外領土、21の全国薬局チェーン、独立した薬局ネットワークが協力して設立され、2021年2月11日以降、COVID-19ワクチンへのアクセスを提供した。今回、2価mRNA COVID-19ワクチン接種の取り組みにおけるFRPPの貢献について、CDCに直接報告されたFRPPの接種データと、各管轄区域の予防接種情報システムに提出された接種データを用いて評価が行われた。アメリカでは2022年9月1日～2023年9月30日、約5,980万回分の2価COVID-19ワクチンが接種され、そのうちFRPPパートナーによる接種は約4,050万回(67.7％)であった。男女ともに2/3以上がFRPPパートナーによる接種であり(男性66.9％、女性68.6％)、年齢別では18～49歳が最も多く(70.6％)、6カ月齢～4歳の乳幼児で最も少なかった(5.9％)。人種/民族別では、非ヒスパニック系アジア人(60.2％)、非ヒスパニック系白人(56.2％)が多く、非ヒスパニック系アメリカ先住民/アラスカ先住民(21.9％)、その他人種(22.3％)にて少なかった。59の管轄区域のうち、5カ所(アメリカ領サモア、北マリアナ諸島、ミクロネシア連邦、マーシャル諸島、パラオ)を除くすべての区域にFRPPパートナーがあり、54管轄区域のうち42区域(77.8％)にて少なくとも半分量の2価mRNA COVID-19ワクチンがFRPPパートナーにより接種された。また、都市部では地方に比べFRPPによる2価ワクチン接種の割合が高く(都市部81.6％、地方60.0％)、都市部では45/52区域(86.5％)、地方では33/48区域(68.8％)にて50％以上がFRPP薬局による接種であった。以上、FRPPパートナーシップによりアメリカにおける2価COVID-19ワクチンの約2/3が接種され、社会人口学的に幅広い層の人にワクチン接種へのアクセスを提供し、このプログラムが定期的ワクチン接種に必要なワクチン接種の取り組み、公衆衛生情の緊急事態時の医療サービスの提供に対応するモデルとして役立つ可能性が示された。
