麻疹排除への進展 ― WHOの東地中海地域、2019年～2022年

2015年、WHOの東地中海地域(EMR)のすべての国および地域である22カ所では2020年までに麻疹排除を達成すると宣言したが、ほとんどの国の地域ではまだ排除に至っていない。今回、前回の報告を更新し、2019年～2022年のEMRにおける麻疹排除への進捗状況について述べる。2019年～2022年の麻疹含有ワクチン(MCV)の初回推定接種率は82％～83％と横ばいであったが、2008年以降では最低の接種率であった。MCV 2回目(MCV2)接種率は2019年の76％から2022年に78％に増加した。MCV2接種率95％以上を達成した国は、2019年の8カ国(36％)から2021年には10カ国(45％)に増加したが、2022年には2019年のレベルまで減少した。2019年～2022年に国内すべての地区でMCV2接種率が95％以上を達成した国は4カ国(18％)のみであった。また、この期間、16カ国に行われた40回の追加予防接種活動にて1億6,000万人が接種した。麻疹サーベイランスの感度が目標値(麻疹および風疹ではないとして処理された10万人あたり2例以上の疑い例を検出)を達成した国は、2019年の14カ国(64％)から2022年には11カ国(50％)に減少し、麻疹疑い例をタイムリーかつ完全に調査するという目標を達成した国は2019年の5カ国(23％)から2022年には10カ国(45％)に増加し、検査のための検体を適切に採取するという目標を達成した国は2019年の21カ国(95％)から2022年には17カ国(77％)に減少し、検体受け取りから4日以内に検査結果が報告された国は2019年の12カ国(55％)から2022年には8カ国(36％)に減少した。報告された麻疹症例数は2019年の22,549例から2020年には7,630例と3分の2に減少したが(66％減)、2021年には倍以上に増加し16,860例となり(121％増)、2022年にはさらに増加し39,266例となった(133％増)。2021年～2022年の麻疹症例数の増加は主にアフガニスタン、パキスタン、イエメンでのアウトブレイクが原因であった。人口100万人あたりの年間麻疹罹患率は2019年の29.8例から2020年には7.4例に75％減少したが、2021年には9.6例(前年比30％増)、2022年には50.0例(前年比421％増)と増加した。100万人あたり5例以上の麻疹罹患率を報告した国の数と割合は2019年の12カ国(55％)から2021年には5カ国(23％)に減少したが、2022年には9カ国(41％)に増加した。EMRにて遺伝子配列が確認された1,345検体からは2つの流行中の麻疹の遺伝子型が検出され、16カ国の1,298検体にて遺伝子型B3、7カ国の44検体にて遺伝子型D8であった。麻疹排除の地域的検証のための委員会は2019～2022年に4回招集され、2019年末までに3カ国(14％、バーレーン、イラン、オマーン)、2021年にはエジプトにて麻疹排除が確認され、2022年末まで維持されていることが確認された。COVID-19パンデミック後、不十分なワクチン接種率、それによる免疫ギャップの拡大、麻疹サーベイランス実施の低下によりのため、EMRでは麻疹患者が増加した。今後、定期的な麻疹ワクチン接種活動、SIAの実施を継続、増加し、タイムリーな症例ベースサーベイランスおよび検査の実施を再び始める必要がある。
