Advisory Committee on Immunization Practicesが19歳以上の成人に推奨するワクチン接種スケジュール ― アメリカ、2024年

2023年10月、Advisory Committee on Immunization Practicesは2023年10月の会議でアメリカの19歳以上の成人における2024年の予防接種スケジュールを承認した。2023年からの変更は、呼吸器抱合体ウイルスワクチン(RSV)、インフルエンザワクチン、COVID-19ワクチン、不活化ポリオウイルスワクチン(IPV)、サル痘ワクチン(Mpox)、髄膜炎菌血清群A、B、C、W、Y 5価ワクチン(MenACWY-TT/MenB-FHbp)の接種が新たに推奨または更新され、髄膜炎菌血清群A、C、W、Y多糖体ジフテリアトキソイド結合型ワクチン(MenACWY-D:メナクトラ)は供給停止のため削除された。その他、A型肝炎ワクチン(HepA)、B型肝炎ワクチン(HepB)、ヒトパピローマウイルスワクチン(HPV)、麻疹/おたふく風邪/風疹ワクチン(MMR)、肺炎球菌ワクチン、破傷風/ジフテリア/百日咳ワクチン(Tdap)の推奨事項の説明が変更された。現在のmRNA COVID-19ワクチンはすべて1価であり、2022年～2023年に使用された2価ワクチンの使用は推奨されなくなり、削除された。すべての成人に対し最新(2023～2024年型)COVID-19ワクチンを少なくとも1回接種することが推奨されている。HepAはワクチン接種が十分ではなく、接種を希望する人への接種が推奨され、HepBは60歳以上に対するリスクベース接種が推奨されている。インフルエンザワクチンに関しては卵アレルギーの既往歴に関する記述を削除、年齢と健康状態に適応した接種が可能となった。Mpoxは成人予防接種スケジュールに追加され、定期的な接種を必要とするリスク因子が記載されている。RSVに関しては、定期予防接種としてアメリカ本土の多くの地域にて9月～1月に妊娠32～36週の妊婦にAbrysvo(Pfizer社製)を使用、さらに乳児RSV下気道感染症の予防のため、母親のRSVワクチン接種か、乳児のRSV-mAb(ニルセビマブ)接種が推奨されることが追加された。接種時期は地域のRSVの季節性に基づく公衆衛生局のガイドラインに従うものとし、また、重症RSV疾患リスクとワクチンの有益性を考慮する場合の判断因子と病状に関する注釈が追加された。
