Advisory Committee on Immunization Practicesが小児および18歳以下の青年期齢に推奨するワクチン接種スケジュール ― アメリカ、2024年

Advisory Committee on Immunization Practicesは2023年10月の会議でアメリカの18歳以下の小児および青年期齢における2024年の予防接種スケジュールを承認した。このスケジュールはワクチンにより予防可能な疾患の疫学および社会的影響、ワクチンの有効性、安全性、エビデンスの質、プログラム実施の実現可能性、健康の公平性への影響、経済分析などの製品関連データを検討した上で策定される。2023年からの変更点として、インフルエンザワクチン、肺炎球菌ワクチン、呼吸器合胞体ウイルスモノクローナル抗体(RSV-mAb)、RSV、COVID-19ワクチン、不活化ポリオウイルスワクチン(IPV)、サル痘ワクチン(Mpox)、髄膜炎菌血清群A、B、C、W、Yワクチン(MenACWY-TT/MenB-FHbp)の接種が新たに推奨または更新され、ジフテリア/破傷風トキソイド沈降ワクチン(DT)、13価肺炎球菌結合型ワクチン(PCV13)、2価COVID-19 mRNAワクチン、髄膜炎菌血清群A、C、W、Y多糖体ジフテリアトキソイド結合型ワクチン(MenACWY-D:メナクトラ)は、供給停止および小児、思春期齢への使用が推奨されなくなったため削除された。その他、ジフテリア/破傷風/百日咳ワクチン(DTaP)、ヘモフィルスインフルエンザb型ワクチン(Hib)、ヒトパピローマウイルスワクチン(HPV)、麻疹/おたふく風邪/風疹ワクチン(MMR)、髄膜炎菌血清群Bワクチン(MenB)の推奨事項の説明が変更された。適応症別のスケジュールが大幅に改訂され、成人のスケジュールと調和するようになった。カバーページには新たに20価肺炎球菌結合型ワクチン(PCV20)、RSVモノクローナル抗体(ニルセビマブ)、妊婦用RSV(Abrysvo)、Mpox(Jynneos)、5価髄膜炎菌ワクチン(MenACWY-TT/MenB-FHbp、Penbraya)が追加された。COVID-19ワクチンは現行の推奨事項を反映するよう改訂され、必要な接種回数、接種間隔は過去の接種歴、免疫不全状態、使用するワクチン製品により異なる。インフルエンザワクチンに関しては卵アレルギーの既往歴に関する記述を削除、重症度に関わらず接種が可能となった。ポリオウイルスワクチンは18歳以下の青少年のほとんどは小児期に接種を受けていると考えてよいとされ、曝露リスクの高い場合のみ、1回限りのブースター接種が可能である。RSV-mAbは乳幼児に対する使用の詳細が記載され、8カ月齢未満の乳児への推奨事項、および重症RSV疾患リスクが高い8～19ヵ月齢の小児に対する推奨事項が記載された。
