メディケアPart D受給者に対する局所抗真菌薬の処方 ― アメリカ、2021年

表在性真菌皮膚感染症の生涯有病率は世界で20％以上と推定されており、特に65歳以上の高齢者に多い。抗菌薬耐性表在性真菌感染症、特に皮膚糸状菌症(白癬)の出現と蔓延により、南アジアでは局所抗真菌薬や一次治療の経口療法が反応しない大規模なアウトブレイクが発生している。この出現および蔓延は局所抗真菌薬、特に抗真菌薬/コルチコステロイド配合クリーム剤の過剰使用や誤用により増幅している可能性があり、アメリカでは少なくとも11州にて抗菌薬耐性皮膚糸状菌症の患者が確認されている。今回、CDCは2021年のメディケアPart Dの受給者(受給者約4,880万人)のデータを使用して、局所抗真菌薬の処方件数、処方率、費用について調査した。2021年のPart D受給者への局所抗真菌薬の合計処方数は約650万件(1,000名あたり134.0件)であり、処方件数別ではケトコナゾール(240万件、36.6％)、ナイスタチン(190万件、29.0％)、クロトリマゾール・ベタメタゾンジプロピオン酸エステル(90万件、14.7％)が多かった。局所抗真菌薬処方の総費用は2億3,100万ドルであり、1処方あたりの平均費用はエフィナコナゾール(1,035.38ドル)、タバボロール(784.63ドル)、オキシコナゾール(729.84ドル)にて高く、ナイスタチン(25.66ドル)、クロトリマゾール・ベタメタゾンジプロピオン酸エステル(27.82ドル)、クロトリマゾール(30.36ドル)、ケトコナゾール(30.69ドル)にて低かった。アメリカ国勢調査局の地域別では、処方率は北東部(受給者1,000名あたり188.0件）、南部(同138.1件)、中西部(同112.2件)、西部(同103.5件)の順で高かった。処方者全体1,017,417名のうち130,637名(12.8％)が局所抗真菌薬を処方し、医師1人あたりの処方数は皮膚科医が最も多く(87.1件)、足専門医(67.2件)、プライマリケア医(12.3件)と続いた。処方数は計650万件であり、プライマリケア医(260万件、40,0％)、診療看護師/診療アシスタント (140万件、21.4％)、皮膚科医(110万件、17.6％)、足専門医(90万件、14.1％)の順であった。今回の調査では2021年、メディケアPart D受給者に対し650万件の局所抗真菌薬の処方が行われ、その費用は2億3,100万ドルであった。局所抗真菌薬の多くは処方箋なしで市販されているため、実際の使用量はさらに多いものと思われる。皮膚科医および足専門医による処方量は少ないが、医師1人あたりの処方率は高く、表在性真菌感染症の患者を頻繁に診察している可能性が考えられる。また、クロトリマゾール・ベタメタゾンジプロピオン酸エステルの処方量が多く、ステロイドによる皮膚障害、視床下部-下垂体-副腎軸の抑制などのリスクを認識し、代替療法を考慮すべきと思われる。
