Influenza and Up-to-Date COVID-19 Vaccination Coverage Among Health Care Personnel — National Healthcare Safety Network, United States, 2022–23 Influenza Season

Advisory Committee on Immunization Practicesは医療従事者(HCP)に対し年1回のインフルエンザワクチンの接種とCOVID-19ワクチン接種を最新の状態に保つよう推奨しており、医療施設はCDCのNational Healthcare Safety NetworkにHCPのワクチン接種率の報告を義務付けられている。今回、2022-23年インフルエンザシーズンにおける急性期病院および介護施設で働くHCPのインフルエンザワクチンおよびCOVID-19ワクチンの接種率を報告する。急性期病院(4,057施設)に勤務するHCP約840万人のインフルエンザワクチン接種率は全体で81.0％であり、非正規雇用の医師/看護師などの有資格者にてもっとも低く(67.2％)、正規雇用者(83.1％)、非正規雇用の学生/ボランティア(85.2％)にて高かった。地域別では中西部にてもっとも高く(84.7％)、太平洋領域にてもっとも低かった(74.4％)。介護施設(13,794施設)で働くHCP約200万人におけるインフルエンザワクチン接種率は全体で47.1％、正規雇用者にて低く(46.1％)、非正規雇用の有資格者(55.3%)、非正規雇用の学生/ボランティア(57.7％)にて高かった。地域別では太平洋地域にて高く(61.1%)、南部にてもっとも低かった(39.7％)。施設の規模、都市部/農村部、社会的脆弱性指数(SVI)による有意な相違は認めなかった。COVID-19ワクチン接種が最新の状態にあるHCPは、急性期病院(4,057施設)に勤務する約770万人において17.2％であり、太平洋地域にてもっとも高く(28.9％)、山岳地帯にてもっとも低かった(9.1％)。その他の因子による相違は認めなかった。介護施設(13,794施設)で働くHCP約160万人におけるCOVID-19ワクチン接種率は22.8%であり、太平洋地域にてもっとも高く(40.7％)、南部にてもっとも低かった(17.5％)。また、都市部(24.2％)に比べ農村部にて低かった(17.5％)。HCPへのワクチン接種は医療現場における感染の伝播を最小限に抑えるため重要な戦略であるが、今回の調査ではHCPにおけるワクチン接種率が低く、関連する要因および接種率を改善するためのアプローチを特定し、職場での無料接種など、エビデンスに基づく戦略の推進が必要である。
