Progress Toward Poliomyelitis Eradication — Pakistan, January 2021–July 2022

ポリオ根絶への進捗 ― パキスタン、2021年1月～2022年7月

パキスタンとアフガニスタンは野生型ポリオウイルス(WPV)の流行性伝播が途絶えたことがない唯一の国である。今回、2021年1月～2022年7月におけるパキスタンのポリオ根絶への進捗状況を報告し、前回の報告を更新する。WHOおよびUNICEFが推算した2021年のパキスタンにおけるポリオワクチン接種率は83％であり[生後12カ月までの経口ポリオワクチン(OPV)3回接種または不活化ポリオワクチン1回接種]、12～23カ月齢の小児における州別でのOPVの3回接種率は45.1％(Balochistan州)～94.9％(Punjab州)であり、Balochistan州、Khyber Pakhtunkhwa州、Sindh州では接種率が80％を超える地区はなく、Punjab州では31/36地区にて80％を超えていた。2021年、5歳未満の小児を対象とした2価OPVによる全国ワクチン接種日(NID)は4回、地域別ワクチン接種日(SNID)は2回実施され、2022年は2回のNID(3月と5月)、2回のSNID(1月と6月)が実施され、さらに2回のNID(8月と11月)と1回のSNID(10月)が予定されている。急性弛緩性麻痺(AFP)サーベイランスでは、15歳未満の小児および思春期齢10万人あたりの非ポリオAFP症例は2021年に13例であったが、2022年では7月27日時点で14.2例であった。また、症例調査の便検体回収率は全国および各州にて2021年、2022年ともに80％以上であった。パキスタンの環境サーベイランスの検体採取地点は77カ所あり、2021年は採取した下水検体833検体のうち65検体(8％)がWPV1型(WPV1)陽性であり(2020年は407/786検体、52％)、2022年ではこれまでに748検体中13検体(2％)が陽性であった(Khyber Pakhtunkhwa州8件、Punjab州4件、イスラマバード1件)。2021年におけるWPV1症例はBalochistan州Killa Abdullah地区より1例が報告され、2020年は数州から84例、2019年は147例の報告であった。2022年は8月20日の時点でKhyber Pakhtunkhwa州から14例のWPV1症例が報告されている(North Waziristan地区13例、Lakki Marwat地区1例)。2021年1月～2022年7月のWPV1症例15例の年齢は生後7～28カ月齢(中央値15カ月齢)、87％はOPV未接種であった。また、伝播型ワクチン由来ポリオウイルス2型(cVDPV2)感染例は、2019年7月～2021年8月にて165例であり(2019年22例、2020年135例、2021年8例)であり、2021年4月23日の発症が最後の症例である。
