2021年8月29日に、アメリカ政府はアメリカ市民とアフガニスタン国民をアフガニスタンからアメリカに安全に再定住させるために、アメリカ国土安全保障省(DHS)が主導し、アメリカ国防総省(DoD)とアメリカ国務省(DoS)が実施したOperation Allies Welcome(OAW)の緊急の設立を監督した。避難者たちはヨーロッパとアジアのいくつかの海外拠点に一時的に収容された後、アメリカの2つの国際空港を経由して軍用およびチャーター便で8つのアメリカ軍事基地に輸送され、ホテルAは特定の感染症の患者と感染症に曝露した人の隔離と検疫に使用された。アフガニスタンでは麻疹のアウトブレイクが継続中であり(2021年1月～11月に全国で臨床例25,988例の報告)、麻疹の定期予防接種率が低いため(2020年の1回目と2回目の接種率はそれぞれ66％、43％)、CDCは8月30日にアフガニスタンの避難者における麻疹に対する警戒の維持を公衆衛生当局者に促すEpi-X通知を発行した。9月4日、CDCはウィスコンシン州フォート・マッコイでのアフガニスタン人避難者において麻疹が疑われる症例に関する通知を受けた。CDCは州および地方の保健部門と協力し、DHS、DoD、DoSに技術的なガイダンスを提供し、麻疹サーベイランスを強化し、症例調査と接触者追跡を実施するとともに、海外および国内の軍事基地とホテルAで麻疹ワクチンの集団接種キャンペーンを実施した。8月29日(OAWの開始)～11月26日までに避難者72,299名のうち、47例の麻疹確定診断症例が報告された。症例は4つの管轄区域にある6施設で報告され(バージニア州22例、ウィスコンシン州22例、ニューメキシコ州2例、ニュージャージー州1例)、発疹の発症日は9月2日～10月15日であった。年齢中央値1歳(0～26歳)で、55％が男性であった。47例全例がアメリカに到着時点でワクチン未接種またはワクチン接種の有無不明であり、46例(98％)が検査で確認され、遺伝子型を調査した43例全例の検体からB3型が同定された。粗発病率は0.065％であった。また、水痘57例、おたふくかぜ14例、風疹1例も報告された。この避難者集団における麻疹・おたふくかぜ・風疹混合ワクチンの接種率推定96％を達成した国内および海外拠点でのワクチン接種の取り組みは、アメリカへの麻疹の輸入を制限し、その後の軍事基地およびアメリカ国内のコミュニティへの拡散を防ぐ上で重要であった。
