Progress Toward Rubella and Congenital Rubella Syndrome Control and Elimination — Worldwide, 2012–2020

風疹および先天性風疹症候群の対策と根絶への進歩 ― 全世界、2012～2020年

風疹ウイルス感染は通常は軽度の熱性発疹を引き起こすが、妊娠中の感染、とくに妊娠第1期では、流産、胎児死亡、死産、出生児に先天性風疹症候群(CRS)として知られる先天性障害が生じる転帰になる可能性がある。風疹は風疹含有ワクチン(RCV)の1回接種により、生涯にわたって予防が可能である。Global Vaccine Action Plan 2011-2020は2020年までにWHOの6地域のうち少なくとも5地域にて風疹根絶を達成することが目標として含まれており、今回、2021年から2020年における風疹およびCRS対策および根絶に関する世界の進捗状況について前回の報告から更新し、要約した。2020年、RCVは194カ国中173カ国(89％)にて定期予防接種スケジュールに取り入れられており、2012年の132カ国(68％)から31％増加した。アメリカ地域(AMR)、ヨーロッパ地域(EUR)、南東アジア地域(SEAR)、西太平洋地域(WPR)ではすべての国でRCVが導入され、アフリカ地域(AFR)では47カ国中31カ国(66％)、東地中海地域(EMR)では21カ国中16カ国(76％)にて導入されている。所得別のRCV導入率は、2012年では高所得国が59カ国すべて、高中所得国が54カ国の91％、低中所得国が54カ国の43％、低所得国が28カ国のわずか4％であったが、2020年までに高中所得国は94％、低中所得国は93％、低所得国は48％に増加した。WHO/UNICEFのEstimates of National Immunization Coverageによる幼児のRCV接種率は全世界にて2012年の40％から2020年には70％に増加したが、2020年の地域差は大きかった[AFR(36％)～WPR(95％)]。また、2020年のRCV接種率は低所得国では26％、低中所得国と高中所得国では76％、高所得国では93％であった。風疹およびCRSサーベイランスデータでは、2020年に194カ国すべてがサーベイランスを実施し、風疹の症例数を報告した国(症例数0例の報告を含む)は2012年の175カ国(90％)から2019年には179カ国(92％)に増加したが、COVID-19パンデミック期2020年では135カ国(70％)に減少した。同様に、CRSの症例数を報告した国は2012年の130カ国(67％)から2019年には131カ国(68％)に増加したが、2020年は112カ国(58％)に減少した。報告された風疹症例数は2012年に4,277例であったのが、2019年には48％減少して49,136例、2020年にはさらに減少して10,194例になり、報告されたCRS症例数は2012年の302例から2020年には603例に増加したが、これは主に、2012年以降に人口の多い国(バングラデシュ、インド、インドネシア、パキスタン)でのCRSサーベイランスおよび報告が開始され、2020年にパキスタンでの報告が変更されたためである。2019年にSEARは風疹およびCRS対策目標を根絶目標に変更し、AMR、EUR、WPR、SEARにて根絶目標が定められ、AFR、EMRはまだ目標を設定していない。風疹流行の根絶は93カ国[AMRは35カ国(100％)、EMRは21カ国中3カ国(14％)、EURは53カ国中49カ国(92％)、SEARは11カ国中2カ国(18％)、WPRは27カ国中4カ国(15％)]にて確認されている。
