Use of Recombinant Zoster Vaccine in Immunocompromised Adults Aged ≥19 Years: Recommendations of the Advisory Committee on Immunization Practices — United States, 2022

19歳以上の免疫不全の成人における組換え帯状疱疹ワクチンの使用：Advisory Committee on Immunization Practicesの勧告 ― アメリカ、2022年

組換え帯状疱疹ワクチン(RZV：シングリックス、グラクソ・スミスクライン社)は、2回接種(各回0.5mL)のサブユニットワクチンで、アジュバント(AS01B)と組み合わせた組換え糖タンパク質Eが含まれ、2017年にFDAにより50歳以上の成人を対象とした帯状疱疹予防としてアメリカで認可され、Advisory Committee on Immunization Practices(ACIP)によって50歳以上の免疫正常の成人に推奨されている。2021年7月23日には、FDAは免疫不全または免疫抑制のため帯状疱疹のリスクが増加または増加の可能性がある18歳以上の成人が含まれるように、RZV使用の適応を拡大した。2021年10月20日に、ACIPは、疾患または治療により免疫不全または免疫抑制となった、またはその可能性のある19歳以上の成人を対象に、帯状疱疹と関連する合併症を予防するため、RZVの2回接種を推奨した。RZVは、免疫不全の人への使用が承認された初めての帯状疱疹ワクチンで、中程度から高いワクチン有効性と許容可能な安全性プロファイルにより、帯状疱疹の罹患率と関連する合併症を予防する可能性がある。今回の報告では、帯状疱疹の予防に関する前回のACIP勧告を更新する。帯状疱疹の既往歴や以前の帯状疱疹生ワクチンの接種歴に関係なく、RZVの2回接種が必要である。2回目のRZV接種は、通常、初回接種後2～6カ月後に行うが、免疫不全または免疫抑制状態にあるか、その可能性にある人、または、ワクチン接種スケジュールを短縮することが有効な人の場合では、2回目接種は初回接種から1～2カ月後に行うことができる。可能であれば、免疫抑制状態になる前にワクチン接種をすべきである。RZVのようなアジュバント添加の組換えワクチンは、COVID-19ワクチンを含む他の成人用ワクチンと一緒に、異なる解剖学的部位に同時投与が可能である。妊娠中のRZV使用に関するACIP勧告は現時点でないため、医療従事者は妊娠終了後までRZVを延期することを考慮すべきである。RZVのような組換えワクチンに授乳中の母親や乳児にもたらす既知のリスクはない。また、RZVのワクチン成分に対してアナフィラキシーなどの重度のアレルギー反応の既往歴のある人に対して投与すべきではない。
