Community-Based Testing Sites for SARS-CoV-2 — United States, March 2020–November 2021

SARS-CoV-2の地域密着型検査施設 ― アメリカ、2020年3月～2021年11月

2020年3月13日にCOVID-19が国家緊急事態と宣言され、その直後にアメリカ政府は疫学的サーベイランス、フロントライン就労者およびCOVID-19感染リスクの高い集団のモニタリングおよび限られた検査資源の特定と配分のための全国検査プログラムを開始した。保健福祉省と連邦緊急事態管理庁が共同で主導するWhite House Joint Task Forceは、州および地域のパートナーとともに地域密着型検査施設(CBTS)プログラムを立ち上げた。今回はCBTSプログラムの予定、提供されるサービス、範囲について記述する。このプログラムは1)州および地域パートナーと協力でのドライブスルー検査、2)政府と薬局など民間パートナーと共同でのファーマシー＋検査、3)州、部族、地域の保健機関のサポートにて検査能力を向上したリスクのある地域での迅速サーベイランスのためのサージ検査の3つで構成され、連邦政府が資金供給し、無料で行われた。2020年3月19日～2021年4月11日のCTBSプログラムでは、アメリカおよびアメリカ海外領土の8,319カ所にて11,661,923件のSARS-CoV-2検査が実施された[ドライブスルー検査402,223件(3.5％)、ファーマシー＋検査10,129,142件(86.9％)、サージ検査1,130,558件(9.7％)]。検査結果は陽性が1,176,959件(10.1％)、陰性が10,430,749件(89.4％)、不確定が54,215件(0.5％)であり、ドライブスルー検査では陽性が59,195件(14.7％)、陰性が337,255件(83.9％)、不確定が5,773件(1.4％)であった。ファーマシー＋検査およびサージ検査では計11,211,258件中1,117,764件(10.0％)が陽性であり、3,746,029件(42.3％)に症状を認め、年齢は20～54歳の成人が67.8％、42.3％に症状があった。人種は9,396,284名(83.5％)にて明らかであり、白人が54.3％(陽性率9.9％)、黒人が11.6％(同10.4％)、アジア人が6.6％(同7.3％)、アメリカインディアン/アラスカ先住民が0.5％(同14.1％)、ハワイ先住民/太平洋諸島民が0.9％(同6.4％)、その他人種が27.5％(同9.8％)であった。民族は6,121,887名(54.4％)で明らかであり、ヒスパニック系が25.3％(陽性率15.9％)であった。年齢別では20歳未満および45～54歳(0.7％)と85歳以上(11.5％)にて検査陽性率が高く、男女別では男性(10.8％)にて女性(9.2％)より高かった。また、症状のある人では17.1％、無症状の人では5.1％が陽性であり、無症状の人では85歳以上(7.4％)、20歳未満(6.3％)にて陽性率が高かった。検体採取から結果報告までの期間は全体では82.0％が2日以内であったが、全国の検査数および症例数がピークであった2020年7月および12月はそれぞれ40.7％、59.3％と低かった。CBTSプログラムにおける陽性率は全国で報告された症例数の増加に伴い増加しており、現在の地域密着型のスクリーニング検査、サーベイランス、疾病管理プログラム、今後の公衆衛生上の緊急事態への対応の指針となるモデルとなっている。
