Racial, Ethnic, and Gender Disparities in Awareness of Preexposure Prophylaxis Among HIV-Negative Heterosexually Active Adults at Increased Risk for HIV Infection — 23 Urban Areas, United States, 2019

HIV感染リスクの高いHIV陰性の異性間性行為を行う成人における曝露前予防に対する認知度の人種、民族、性別による格差 ― 23都市部、アメリカ、2019年

2019年、アメリカおよび6管轄地域におけるHIV新規診断例の23％を異性間性行為者が占めていたことから、CDCはNational HIV Behavioral Surveillanceデータを分析し、HIV感染率の高い23の都市部にいる異性間性行為を行う成人を対象にHIV曝露前予防(PrEP)に関する認知度調査を行った。調査は対面式の聞き取り調査とHIV検査により実施し、対象者は計9,359名であった。PrEPについて認知していたのは3,026名(32.3％)であり、男性が,221名(29.2％)、女性が1,805名(34.8％)であった。男性では黒人の31.9％、白人の24.2％、ヒスパニック系の19.5％、女性では黒人の40.3％、白人の32.7％、ヒスパニック系の17.6％がPrEPについて認知しており、男女ともにヒスパニック系にてPrEPの認知度が低かった。医療保険未加入者(26.4％)は加入者(34.2％)に比べ、かかりつけ医のいない人(29.1％)はいる人(34.3％)に比べ認知度が低かった。また、出生地別ではプエルトリコ(8.2％)、メキシコ(12.6％)の人ではアメリカ50州およびワシントンDCの人(35.1％)に比べてPrEPの認知度が低く、英語を母国語としないアメリカ以外の国で生まれた人はアメリカ生まれの人に比べPrEPの認知度が低かった(6.5％ vs. 35.2％)。地域別では北東部33.2％、中西部37.6％、南部36.0％、西部28.8％、プエルトリコ5.8％であり、南部に比較して、プエルトリコにて低かった。個々似合わせたPrEPキャンペーンや定期的スクリーニングにより、PrEP認知度、異性愛者成人、特にヒスパニック系における利用が向上する可能性がある。
