Use of Pfizer-BioNTech COVID-19 Vaccine in Persons Aged ≥16 Years: Recommendations of the Advisory Committee on Immunization Practices — United States, September 2021

Pfizer社/BioNTech社製のCOVID-19ワクチン(BNT162b2)は、脂質ナノ粒子により製剤化されており、COVID-19の原因ウイルスであるSARS-CoV-2の融合前スパイク糖タンパク質をコードするヌクレオシドmRNAワクチンであり、接種は3週間間隔で2回の筋肉内投与する(30μg、各0.3 mL)。2020年12月に、ワクチンはFDAから緊急使用許可(EUA)が認可され、Advisory Committee on Immunization Practices(ACIP)により16歳以上に使用するための暫定推奨が出された。2021年5月には、Pfizer/BioNTech社製COVID-19ワクチンに関するEUAおよび暫定ACIP推奨は、12～15歳の青年に拡大された。2020年12月14日～2021年9月1日に、アメリカでは約2億1100万回分のPfizer社/BioNTech社製COVID-19ワクチンが投与された。2021年8月23日にFDAは、16歳以上にPfizer社/BioNTech社製COVID-19ワクチン(Comirnaty、Pfizer社)を使用するための生物製剤承認申請を承認した。Pfizer社/BioNTech社製COVID-19ワクチンのエビデンスに関するACIP COVID-19 Vaccines Work Groupの結論は、2021年8月30日の公開会議でACIPに提示された。Pfizer社/BioNTech社製COVID-19ワクチンに関する審議を導くために、ACIPはEvidence to Recommendation Frameworkを使用し、Grading of Recommendations, Assessment, Development and Evaluation(GRADE)アプローチを取り入れた。GRADEのエビデンス評価から、16歳以上に対するPfizer社/BioNTech社製COVID-19ワクチン接種の有益性に関する確実性レベルは、症候性COVID-19の予防に対してはタイプ1(高い確実性）、COVID-19に起因する入院および死亡の予防に対してはタイプ2(中程度の確実性)、無症候性SARS-CoV-2感染の予防に対してはタイプ4(非常に低い確実性)であった。ワクチン接種後に可能性がある有害性に関するエビデンスは、重篤な有害事象に対してはタイプ2、反応原性に対してはタイプ1であった。初期の臨床試験データに加えて、ACIPはPfizer社/BioNTech社製COVID-19ワクチンに対する暫定推奨の発行から8カ月間に収集された臨床試験での追加の観察期間、実臨床でのワクチン有効性研究、認可後のワクチンの安全性モニタリングなどの新しい情報についても検討した。追加情報により、無症候性感染、COVID-19、関連する入院と死亡の予防から得られる有益性は、ワクチン関連リスクを上回るという確実性が高まった。2021年8月30日に、ACIPはCOVID-19の予防のために、16歳以上に対するPfizer社/BioNTech社製COVID-19ワクチンの使用を推奨した。
