National, Regional, State, and Selected Local Area Vaccination Coverage Among Adolescents Aged 13–17 Years — United States, 2020
13~17歳の思春期齢における全国、地域、州、特定地域におけるワクチン接種率 ― アメリカ、2020年
CDCは2020年のNational Immunization Survey-Teenデータを分析し、13~17歳の思春期齢20,163名における推奨ワクチンの接種率を推定した。ヒトパピローマウイルス(HPV)ワクチンの1回以上接種率は2019年の71.5%から2020年には75.1%に増加した。HPVワクチン接種を予定通りにしている(HPV UTD)基準は3回以上接種、あるいは1)2回接種、2)1回目を15歳の誕生日前に接種、3)1回目接種と2回目接種との間隔が5カ月-4日以上、のいずれかに該当であり、HPV UTD思春期齢の割合は2019年の54.2%から2020年には58.6%に増加した。2020年の破傷風・ジフテリア・無細胞性百日咳(Tdap)ワクチン1回以上の接種率は90.1%、髄膜炎菌結合型(MenACWY)ワクチン1回以上(17歳は2回以上)の接種率は89.3%(2回以上54.4%)と2019年と同等であった(2019年の接種率はそれぞれ90.2%、88.9%、53.7%)であった。また、13~17歳におけるA型肝炎ウイルス(HepA)ワクチン2回以上接種率は2019年の77.1%から2020年には82.1%、17歳の血清群B群髄膜炎菌ワクチン1回以上接種率は2019年の21.8%から2020年に28.4%とともに増加した。麻しんおたふくかぜ風しん混合ワクチン2回以上接種率、B型肝炎ウイルスワクチン3回以上接種率、水痘の既往のない人における水痘ワクチン接種率はいずれも90%を超えていた。大都市統計地域(MSA)以外に住む思春期齢ではMSAに住む思春期齢に比べ、MenACWYの1回以上接種率(85.7% vs. 90.2%)、HPVワクチン1回以上接種率(68.0% vs. 77.8%)、HepAワクチン2回以上接種率(76.2% vs. 83.6%)、HPV UTD(49.2% vs. 60.4%)が低く、これらの格差は貧困水準以上の家庭の思春期齢にてあり、貧困水準以下の家庭では有意差は認めなかった。また、2020年3月13日に国家緊急事態として宣言されたCOVID-19パンデミックがワクチン定期予防接種に及ぼす影響を評価した。歴史的にHPVワクチン接種率は他のワクチン接種率に比べ低く、13~17歳に対するキャッチアップ接種が認められている。4月の時点で2019年に比べ2020年のHPVワクチン開始率は少なく、8月および9月に差が両年の差は最も大きくなったが、11月末には有意差はなくなった。他のワクチン接種では推奨される年齢を過ぎた13歳以上を対象とした調査であったため、COVID-19パンデミックの影響は認めなかった。
