Trends in Nonfatal and Fatal Overdoses Involving Benzodiazepines — 38 States and the District of Columbia, 2019–2020
ベンゾジアゼピン系薬が関与する非致死的および致死的な過剰摂取の傾向 ― 38州およびワシントンDC、2019年~2020年
非致死的および致死的な薬物過剰摂取は、2019年から2020年にかけて全体的に増加した。違法なベンゾジアゼピン系薬(エチゾラム、フルアルプラゾラム、フルブロマゾラムなど)は、2020年には検死検体および臨床検体における検出が増加し、しばしばオピオイドとともに検出され、薬物の過剰摂取の全体的な増加に寄与した可能性がある。複数の州におけるベンゾジアゼピン系薬が関与する非致死的な過剰摂取、過剰摂取における違法なベンゾジアゼピン系薬の関与に関する最近の傾向のデータは入手が限られている。このデータギャップに対し、2019年1月~2020年12月に32州およびワシントンDCの救急科(ED)で治療されたベンゾジアゼピン系薬の関与が疑われる非致死的過剰摂取(ベンゾジアゼピン過剰摂取によるED受診)、2019年1月~2020年6月に23州にて違法ベンゾジアゼピン系薬および処方ベンゾジアゼピン系薬が関与する過剰摂取による死亡(ベンゾジアゼピンによる死亡)の1年毎および四半期毎の傾向を分析することにより対処した。2019年1月~2020年12月に33管轄区域(32州およびワシントンDC)で報告されたED受診1億1700万件(全ED受診の72%)を分析対象とした。このうちベンゾジアゼピン系薬過剰摂取によるED受診は31,377件が確認され(2019年15,547件、2020年15,830件)、6,883件(21.9%)はオピオイドも関与していた。2020年のベンゾジアゼピン系薬過剰摂取によるED受診は女性(51.5%)および25~34歳(20.9%)にて多く、4分の1近く(22.8%)でオピオイドも関与していた。2019年から2020年にかけて、ED受診10万件あたりのベンゾジアゼピン系薬過剰摂取によるED受診は23.7%増加し、オピオイド関与ありのED受診率はオピオイド関与なしの受診率よりも高い割合であった(34.4% vs. 21.0%)。2020年1月~6月におけるベンゾジアゼピン系薬による死亡の大半(92.7%)がオピオイドに関与し、違法製造フェンタニルの関与も多かった(66.7%)。2019年4月~6月期から2020年4月~6月期にかけて、ベンゾジアゼピン系薬による死亡数は全体で42.9%増加し(1,004例から1,435例)、処方ベンゾジアゼピン系薬による死亡は21.8%(921例から1,122例)、違法ベンゾジアゼピン系薬による死亡は519.6%(51例から316例)増加した。薬物の使用および誤用の一次予防に加え、ナロキソンが利用できる度合いの改善、ベンゾジアゼピンやオピオイドを使用している人の治療アクセスの向上、ベンゾジアゼピン系薬やオピオイドが関与する過剰摂取に対する救急医療要請を強化することで、疾病率および死亡率を減らすことができる。
