Alternative Methods for Grouping Race and Ethnicity to Monitor COVID-19 Outcomes and Vaccination Coverage
COVID-19の転帰およびワクチン接種率を観察するための人種および民族で分類する代替法
人種および民族別のCOVID-19データの人口に基づく分析により、COVID-19の転帰とワクチン接種率における格差を特定し、観察することが可能となる。CDCは格差を特定し、ワクチンなどの予防手段への公平なアクセスを確保するために、人種および民族に関する情報を収集することを推奨しているが、CDCに報告されたCOVID-19データでは、この情報が欠けていることが多い。Office of Management and Budget’s Standards for the Classification of Federal Data on Race and Ethnicity(Statistical Policy Directive No.15)のベースラインデータ収集要件には、2つの民族カテゴリーと最低5つの人種カテゴリーが含まれている。CDCはCOVID-19の症例とワクチン接種の明らかなデータを使用し、人種および民族による分類を、現在の民族性を優先する方法(政策の指示で調整)と民族の情報が欠落している時に人種情報を使用した2つの代替方法(A法およびB法)を比較した。A法では民族データが不明または欠落している場合には非ヒスパニック系民族と仮定し、現在の方法と同様の集団分類(分母)を割合計算に使用した(ヒスパニック系集団および人種カテゴリーではヒスパニック系、これと異なる人種分類では非ヒスパニック系)。B法では現在の方法およびA法とは異なり、相互に排他的ではない民族および人種のカテゴリーに分類し、B法を使用した割合計算の分母は、ヒスパニック系集団に対してはヒスパニック系、これと異なる人種集団に対してはヒスパニック系または非ヒスパニック系とした。現在の方法と比較して、代替法では各人種カテゴリー[アメリカインディアン/アラスカ先住民(AI/AN)、アジア人、黒人/アフリカ系アメリカ人(黒人)、ハワイ先住民/他の太平洋諸島民(NH/PI)、白人]にわたり、COVID-19症例数およびワクチン完全接種完了者数が増加した。B法を使用した場合、症例数の相対的増加はAI/AN(58.5%)で最も大きく、ワクチン完全接種完了者数の相対的増加はNH/PIで最も大きかった(51.6%)。現在の方法と比較して、A法では5つの人種集団での累積罹患率とワクチン接種率が増加し、B法では2つの集団(AI/ANおよびNH/PI)での累積罹患率が減少し、AI/ANでの累積ワクチン接種率が減少した。白人と比較した人種および民族別のCOVID-19症例の割合は方法によって異なっていたが、3つの方法すべてでアジア人は1未満、その他の集団では1を超えていた。ワクチン完全接種を完了する傾向は、3つの方法すべてでNH/PIにて最も高かった。今回の分析は、データが不完全な場合に人種および民族データを分析するための代替方法が、格差に関して異なる結論を導く可能性があることを示している。これらの方法には制限があるが、人種や民族のデータが不完全な場合に格差を分析および追跡するための追加の方法を特定するために、潜在的なバイアスの調査と専門家との相談がさらに必要である。
