HIV Testing Trends Among Persons with Commercial Insurance or Medicaid —United States, 2014–2019
民間医療保険またはメディケイドの加入者におけるHIV検査の傾向 ― アメリカ、2014年~2019年
効果的なHIVの予防および治療にはHIV検査が重要な要素である。CDCは13~64歳の性的に活発なすべての人に対し、生涯に少なくとも1回は医療機関にてHIV検査を受けることを推奨しているが、最近の研究では、医療現場でのHIV検査率が低いことが示されており、2009年~2016年、HIV検査率は上昇しなかった。今回、2014年~2019年の民間医療保険またはメディケイドの加入者におけるHIV検査の傾向を推定し、2019年の人口統計学的特徴について説明する。IBM MarketScan Commercial Claims and Encountersデータベース(民間医療保険)とCenters for Medicare & Medicaid Services(CMS)医療費請求データベース(メディケイド)の加重データを分析し、HIV検査を受けた人の割合を推定した。結果、2014~2019年において、13歳以上の男性および妊娠していない女性におけるHIV検査率はどちらの保険加入者でも上昇していた。1年あたりの上昇率は民間医療保険で6.0%、メディケイドで3.2%であり、メディケイド加入者ではヒスパニック系を除く人種/民族にて上昇傾向を認めたが、2019年にHIV検査を受けたのは民間医療保険加入者の4.0%、メディケイド加入者の5.5%のみであった。検査をした人の割合は65歳以上を除き、すべての地域でメディケイド加入者にて民間医療保険加入者よりも高く、2019年のメディケイド加入者におけるHIV検査率は、非ヒスパニック系黒人(8.5%)、ヒスパニック(5.9%)にて、非ヒスパニック系白人(3.9%)、非ヒスパニック系アジア人(5.0%)よりも高かった。アメリカのEnding the HIV Epidemicのイニシアチブの目標である2025年までにHIV感染者の95%以上が自身の感染を認識していることを達成するには、年間のHIV検査率を少なくとも3倍に上昇させ、数年間それを維持する必要がある。また、人種/民族間の格差を減らすためにも検査率の上昇は必須であり、公衆衛生機関は医療システムと連携して検査の増加を支援する介入を行うべきである。
