Persistent Racial/Ethnic Disparities in Fatal Unintentional Drowning Rates Among Persons Aged ≤29 Years — United States, 1999–2019

1999年~2019年、アメリカでは合計81,947人の不慮の溺死が発生した。溺死は、29歳以下の人の不慮の傷害による死亡の主な原因の1つであり、1~4歳の幼児では、先天性欠損症を除き、他のどの原因よりも多い死因である。溺死率は、1990年以降は減少しているが(世界で57%、アメリカで32%の減少)、溺死リスクの人種/民族格差のため、特定の人種/民族、特にアメリカインディアン/アラスカ先住民(AI/AN)、黒人/アフリカ系アメリカ人(黒人)での溺死率は高いままである。溺死率の減少が人種/民族格差の縮小を伴うかどうかを評価し、年齢層と状況ごとにこれらの格差をさらに記述するために、CDCは1999年~2019年のアメリカの死亡率データを分析した。1999年~2019年に、アメリカでは29歳以下の計34,315人が不慮の溺死により死亡した。調査期間中、溺死の粗死亡率の5年間の移動平均は人口10万人あたり1.5人から1.2人減少した。1999年~2019年に、AI/ANとヒスパニック系を除き、各人種/民族グループの年間溺死率は有意に減少した。年間溺死死亡率が最も高かったのは、AI/AN(1.8~3.6)と黒人(1.6~2.5)であった。白人を基準とした溺死の格差の率比(RR)の5年間の移動平均は、AI/ANでは1.8~2.2、黒人では1.3~1.6であった。白人と比較した黒人のRRは、1999年から2005年にかけて有意に減少し、その後、2005年から2019年にかけて有意に増加した。1999年から2019年にかけて、白人と比較したAI/ANおよびアジア人/太平洋諸島(A/PI)のRRに有意な変化はなかった。白人と比較したヒスパニック系のRRは、1999年~2015年に有意に減少し、2015年~2019年には有意な変化はなかった。白人におけるすべての状況、年齢、年数を結合した溺死率全体と比較した溺死率は、AI/ANでは2.0倍、黒人では1.5倍高く、ヒスパニック系およびA/PIでは低かった。溺死率は、持続的に人種/民族格差に関連しており、溺死の格差に関与する要因のさらなる理解が必要である。不均衡な影響を受けた人種/民族における基本的な水泳や水辺での安全スキルの促進を含む地域ベースの介入を実施および評価することは、溺死の格差を減少させるのに役立つ可能性がある。
