風疹根絶への進展 ― WHOヨーロッパ地域、2005年～2019年

2005年、WHOヨーロッパ地域(EUR)の地域委員会は、麻疹、風疹、先天性風疹症候群(CRS)の地域での根絶を求める決議を可決した。2010年にはEURの全53カ国がこれを再確認し、European Vaccine Action Plan 2015-2020の目標の1つに組み込まれた。風疹は通常、軽度の熱性発疹が現れる疾患で、先天性障害の主な原因になるがワクチンで予防可能である。妊娠中に風疹に感染すると流産、胎児死亡、または視覚、聴覚、心臓のいずれか1つ以上に障害を認めるCRSとして知られる先天性異常を来す。2009年以降、EURの全53カ国は小児の定期予防接種スケジュールに、麻しんおたふくかぜ風しん混合ワクチン、麻しんおたふくかぜ風しん水痘ワクチンなど混合ワクチンの一部として風疹を含むワクチン(RCV)の2回接種を組み込んでいる。2005年～2019年におけるRCVの1回目接種(RCV1)の推定接種率は93～95％であり、2019年は30カ国(57％)にてRCV1の接種率95％以上を達成した。2005年におけるRCVの2回目接種(RCV2)の推定接種率は76％で、2019年は91％に増加した。2019年の国別RCV2推定接種率は76～99％であった。風疹サーベイランスデータはヨーロッパの各国から直接、またはヨーロッパのCentre for Disease Prevention and Controlを介して毎月WHOに報告されている。EURにおける人口100万人あたりの風疹の年間罹患率は2005年の234.9例(206,539例)から2019年には0.67例(620例)に減少した。2019年の人口100万人あたりの風疹罹患率はポーランドが7.7例(292例)、ウクライナが3.1例(138例)で高かった。最後の風疹アウトブレイクは2012年にルーマニア(1,873例)、2013年にポーランド(38,548例)にて報告され、2013年～2019年には風疹症例は大幅に減少し、散発的な症例や小規模クラスターが発生した。CRS症例は2005年に7カ国から16例報告され、2019年に報告されたのは8例のみであった(アゼルバイジャン1例、ルーマニア7例)。2005～2019年、EURからWHOに報告された風疹ウイルスのジェノタイピングは143件のみであり、全国的/地域的な風疹ウイルスの流行性伝播の根絶を判断するにはデータが不十分であった。2011年に設立されたThe　European Regional Verification Commission for Measles and Rubella Eliminationによる評価では、2019年末までに45カ国(85％)にて36カ月以上の持続的中断な伝播の遮断を認め、風疹ウイルスの流行性伝播の根絶が実証された。
