Excess Death Estimates in Patients with End-Stage Renal Disease — United States, February–August 2020
末期腎臓病患者の超過死亡の推定 ― アメリカ、2020年2月~8月
末期腎臓病(ESRD)は、生命を維持するために腎代替療法が必要となるなど、腎機能が恒久的に低下する状態である。アメリカにおけるESRD患者の死亡率は2001年以降定期しているが、COVID-19パンデミック中では、ESRD患者はCOVID-19関連の疾病率および死亡率のリスクが高くなり、その原因の一つとして、免疫システム弱体化と複数の併存疾患がある。ESRD National Coordinating Center(ESRD NCC)は、データの分析、ベストプラクティスの普及、教材の作成を通じて、Centers for Medicare & Medicaid ServicesおよびESRD Networksを支援している。Consolidated Renal Operations in a Web-Enabled Network(CROWNWeb)は、アメリカにおけるMedicare認定の透析施設および腎移植センターで治療を受けている慢性透析中または腎移植を受けるESRD患者に関するデータの収集と情報の維持を容易にするシステムであり、ESRD NCCは、CROWNWebに報告された死亡を分析した。超過死亡の推定値は、2020年2月1日から8月31日までの月間死亡数の観測値と予測値を比較することによって得られた。予測される死亡は、2016年1月1日から2019年12月31日までのデータに基づいてモデル化された。この分析では、2020年2月1日から8月31日にESRD患者1,000例あたり8.7~12.9例の超過死亡、またはESRD患者798,611例の集団において合計6,953~10,31例の超過死亡が推定された。サブグループ分析では、全国の透析患者1,000例あたり推定10.8~16.6例の超過死亡が発生し、腎移植患者1,000例あたり推定2.6~5.5例の超過死亡が発生した。これらの結果は、パンデミック初期段階でのESRD患者の死亡が、前年のデータに基づく予想を上回っていたことを示唆している。COVID-19ワクチン接種、感染コントロールガイダンス、患者教育のような介入の計画および実施中には、ESRD患者を含む超過死亡の地理的および時間的パターンを考慮する必要がある。これらの調査結果は、データに基づいた技術支援と、ESRD患者の超過死亡の原因とパターンのさらなる分析の重要性を強調している。
